[?] A summary of the violations found during the inspection are listed below.

3717-1-03.2(J) / Food contact with equipment and utensils
Critical Food in contact with an unclean surface or linens. Observed the raw chicken wings stacked in a giant pile inside the meat display case. The chicken wings were in direct contact with the display case interior. Food can only contact surfaces that are properly washed, rinsed, and sanitized. Foods must be protected from contamination during display. Corrective Action: The amount of chicken wings on display was reduced. Chicken wings on display were contained within cleaned and sanitized pans.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Repeat Equipment food-contact surfaces or utensils are unclean. Observed the ice machine with a build-up of a mold like substance on the interior. Clean and sanitize the entire ice machine from the top down. Replace the water filter on the ice machine at the same time. Corrective Action: Discard all of the ice in the ice machine and then clean and sanitize the entire machine top to bottom. Discard all of the ice that is currently in the machine.

3717-1-07.2 / Poisonous or toxic materials: storage and display - separation
Critical Repeat Improper display of poisonous or toxic materials offered for retail sale. Observed plastic wrap, plasticware, and food storage bags stored under chemicals on the isle display shelf. Store toxic or poisonous materials under all single service items on the isle shelves and throughout the facility. Single service items must be protected from contamination during display.

3717-1-04(A) / Multiuse utensils and food contact surfaces - material characteristics.
Critical Equipment and/or utensils improper construction. Observed the facility using old raw chicken boxes to store other foods in. All food contact surfaces must be constructed so that they are smooth and easily cleanable. Provide commercial NSF approved containers for proper food storage. Cardboard must be discarded once empty of original contents. :Corrective Action: Facility started removing the chicken boxes during the inspection and will continue until they have all been removed today.

3717-1-06.2(C) / Handwashing sinks - hand drying provision.
Repeat No towels or drying device at the handwashing sink(s). Provide paper towels in at the handsink in the rest-room at all times. All handwashing sinks must be stocked with soap and paper towels at all time to allow for proper handwashing.

3717-1-03.5(C)(1) / Food labels - packaged food
Repeat A food packaged in the facility not labeled. Observed the facility packaging bagged ice and corn on the cob (frozen) without any labels. All food packaged on-site must be labeled with the following information: Name and physical address of the facility, list of ingredients and sub-ingredients in order of descending weight, net weight of product, all allergens declared.

Repeat Packaged food labels are not in compliance with standards of identity. Observed the facility offering sleeves of cookies and crackers for retail sale. All manufactured prepackaged foods must have the manufacturers label on each unit in order to be sold individually. Cookies and crackers were removed from retail sale.

3717-1-03.5(A) / Standards of identity
Repeat Packaged food labels are not in compliance with standards of identity. Observed prepackaged bags of frozen onion rings , chicken patties, and french fries offered for retail sale without the manufacturers label. All prepackaged manufactured foods must be properly labeled with the entire manufacturers label.

3717-1-06.1(M) / Outer openings - protected.
Repeat Corrected During Inspection Outer opening not protected. Observed the front door to the facility open during the inspection. All exterior doors must remain closed or screened to prevent the entrance of pests. Screens must open outward and be 16 mesh to one inch. Keep all exterior doors closed to prevent the entrance of pests. Observed exterior fans built into the walls of the facility not screened to prevent the entrance of pests. Screen all exterior opening with 16 mesh to one inch screening.

3717-1-03.2(Y) / Miscellaneous sources of contamination.
Food not protected from contamination. Observed the meat display case leaking/dripping condensate onto the pans of raw chicken wings stored below. Foods are not to be stored below the condensate leak. Chicken wings were removed and empty pans were placed under the leak. Service call was made to repair the unit so that it no longer leaks condensate. Do not store any foods below the leak.

3717-1-04.4(S)(1) / Single-service articles and single-use articles - use limitation.
Re-use of single-service or single-use articles. Observed the facility using black garbage bags to cover the foods on display in the meat case over night. Black garbage bags are not to be used in contact with food. Provide food grade bags for covering the products. The bags are single use and must be discarded each morning. Bags were discarded at the time of the inspection.

3717-1-04.4(S)(1) / Single-service articles and single-use articles - use limitation.
Re-use of single-service or single-use articles. Observed the facility re-using raw chicken boxes for the storage of other foods. Chicken boxes are to be discarded immediately once empty of the original chicken that came in the box. Provide commercial NSF approved storage containers for food storage/display. All chicken boxes were emptied and removed at the time of the inspection.

3717-1-04.4(S)(1) / Single-service articles and single-use articles - use limitation.
Re-use of single-service or single-use articles. Observed the facility re-using a single service container that another food item can in to store the ice scoop at the ice machine. Single service items cannot be washed and reused. Once empty of original contents containers are to be discarded. Provide a commercial NSF approved container for storing the ice scoop in between uses.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Equipment components are not intact, tight or properly adjusted. Observed the light in the meat display case being held up with a packing strap. Repair the light so that it is attached to the cooler as originally designed. Maintain all equipment in good repair throughout the facility.

3717-1-04(J) / Single-service and single-use articles - characteristics.
Single-use articles made of non-food grade materials. Observed the facility covering the foods in the meat display case with black garbage bags at night. Black garbage bags are not intended to be in direct contact with foods that are intended for human consumption. Provide food grade bags for covering foods. Garbage bags were discarded at the time of inspection.

Nonfood-contact surfaces constructed of unapproved materials. Observed newspapers lining the shelves of the produce cooler. Remove the newspaper. Shelves must be smooth and easily cleanable.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed duct tape on the interior shelf of the beer walk-in cooler. Remove the duct tape and repair/replace the shelf. Maintain all equipment in good repair.

Equipment components are not intact, tight or properly adjusted. Observed duct tape on the beer walk-in cooler display shelf. Remove the duct tape as it is not smooth and easily cleanable. Have the shelf repaired or replaced.

Nonfood-contact surfaces constructed of unapproved materials. Observed the facility lining the shelves of the walk-in coolers with cardboard. Remove all cardboard from the shelves. Shelves must be smooth and easily cleanable. Cardboard must be discarded once empty of original contents.

Equipment components are not intact, tight or properly adjusted. Observed the Minute Maid cooler with exposed electrical and peg boards on the interior of the unit. Replace the Minute Maid cooler as it not longer meets the requirements for smooth and easily cleanable or maintained in good condition. Provide make and model number for any replacement cooler to the health department for pre-approval.

Repeat Equipment components are not intact, tight or properly adjusted. Observed one of the sliding glass doors on the meat display cooler with broken glass. Note the other two doors that were broken in the past have been replaced. This is a new door that is now broken. Replace the broken glass in the meat display case.

Equipment components are not intact, tight or properly adjusted. Observed the meat display case leaking condensate into the interior of the unit. Have the meat display case repaired so that it no longer leaks condensate into the interior of the unit. Foods are not to be stored under the leak. All foods were moved at the time of this inspection.

Equipment components are not intact, tight or properly adjusted. Observed peeling paint on the walls of the meat walk-in cooler. Scrape and then repaint the walls of the cooler.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Non-food contact surfaces of equipment are unclean. Observed the prep table that the meat grinder sits on in the walk-in cooler to be filthy. Thoroughly clean the prep table and then maintain it clean.

Non-food contact surfaces of equipment are unclean. Thoroughly clean the top of the ice cream freezer located by the front door. Observed a build-up of filth on the top.

3717-1-05.4(B) / Outdoor storage surface.
Repeat Outdoor refuse storage surface not properly constructed and sloped to drain. Observed that the trash and grease dumpsters are no longer on the asphalt surface. The dumpsters must be located on the asphalt surface to allow for proper cleaning and maintenance. Have the dumpsters placed back on the asphalt surface.

3717-1-05.4(P) / Maintaining refuse areas and enclosures.
Improperly cleaned storage area for refuse, recyclables, or returnables. Observed trash and debris around the dumpsters. Clean up around the dumpsters and then maintain the area clean.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Facility not maintained clean. Observed the entire facility in need of a deep cleaning and organizing. Clean and organize the facility.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Unnecessary or nonfunctional items and /or litter on premises. Observed unnecessary and nonfunctional items throughout the facility. Remove everything that is not necessary to the running of the business. Remove all equipment that no longer functions.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed the wall at the entrance to the meat counter area not in good repair or smooth and easily cleanable. Repair the wall with mortar and then paint it. Wall must be smooth.

Repeat Facility not maintained clean. Observed a build-up of dust and dirt under the shelves in the isles. Thoroughly clean under the isle shelves and then maintain the area clean.

3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings not smooth and easily cleanable. Observed the facility making barriers/walls out of cardboard. Remove all of the cardboard that was hung up behind the prep top cooler in the main cooking area. Walls and barriers must be smooth and easily cleanable.

Repeat Facility not maintained clean. Observed the flour storage room in need of a deep cleaning and organizing. Remove all unnecessary items from the facility.

3717-1-06.1(I) / Light bulbs - protective shielding.
Repeat No protective shielding on lights. Shield the bulb in the pop storage room or provide a plastic bulb. All lights in food prep, display, and storage areas must be shatterproof or properly shielded.

Facility must supply make and model numbers for new equipment prior to purchase/installation. All new equipment must be approved by the health department. Facility is considering replacing several coolers.
