3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Corrected During Inspection TCS foods not being cold held at the proper temperature. Observed portioned ham and cheese sitting out on the counter 55F. This food was recently prepped. Food handler had moved on to a different task. To prevent the growth of bacteria - place TCS food in cold holding when switchin to a different task to maintain proper temperatures. Reviewed proper food handling with PIC.

3717-1-02.4(A)(2) / PIC - Level Two Certified Manager
Repeat Facility does not have a person with level two certification in food protection.

3717-1-04.8(E)(1) / Single-service and single-use articles - storage
[52] Improper storage of single-service and single-use articles. Observed plastic forks stored with the mouth end up. To prevent contamination, store utensils with the handle facing upwards.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed build-up present inside the microwave and underneath the cabinets (front wall hand-sink, drive through window cabinet where salt is stored).

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Observed the three compartment sink leaking into a bucket below. Make necessary repairs and maintain in good order.

Reviewed level two food safety certification with Chris Mowers on previous inspection - each licensed facility is required to have assigned level two certified PIC. This person can not be the same for more than one store. More staff is required to have approved food safety certification.
Hair restraint rule - changes to the Ohio Uniform Food Safety Code may include hair coverings.
