3717-1-02.4(C)(14) / Person in charge: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical Repeat Person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. Observed need for owner to install and maintain this program. Shared handout: Conditional/Foodservice Employee Reporting Agreement
To prevent transmission of foodborne illness, the person in charge shall ensure that food and conditional employees are informed in a verifiable manner of their responsibility to report information about their health as it relates to diseases that are transmissible through food.

3717-1-02.4(C)(16) / Person in charge: duties - ensure the facility has written procedures for vomiting and diarrheal events.
Repeat Person-in-charge did not ensure that the facility has written procedures for employees to follow when responding to vomiting or diarrheal events.. Observed need for owner to install and maintain this program.
Person in charge shall ensure that the FSO or RFE have written procedures for employees to follow when responding to vomiting or diarrheal events that involved discharge onto surfaces.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Equipment and/or components are not maintained in good working order. Observed rust stream from hot faucet handle down into bowl at prep sink.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-06.1(I) / Light bulbs - protective shielding.
Observed light bulbs or heat lamp that were not properly shielded or coated where required. Observed one of the ceiling light tube sleeves with an inch gap between it and the sealing boot.
Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles and does not apply in areas used only for storing food in unopened packages. An infrared or other heat lamp shall be protected against breakage by a shield surrounding and extending beyond the bulb so that only the face of the bulb is exposed.
3717-1-06.4(A) / Repairing.
The physical facilities are not being maintained in good repair. Floor tile at prep sink needs repair/replace.

Level Two training (16 hours needs to be taken). Staff is scheduled for May class.

Consulted on handwashing (did demonstration)-no jewelry, do not wash raw meats!, thaw raw meats correctly. Shared Refrigeration Safety handout...do not let egg contaminate milks or raw chicken contaminate raw bacon.

Temps are good: 41 F or less for refrigerated, 10 F or less for frozen, hot holding at 135 F or higher (PIC resolved issued issue with hot holding cheese topping. Sanitizer is too strong, want 50 ppm to 100 ppm chlorine. Tools are present.

Discussed dirty inside of 2 inch air gap at prep sink (do not have this waste back up or move up into prep sink), used magnetic knife rack to show how it can contaminate clean knives. Good date marking practices, tools are present.
