3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Repeat Equipment food-contact surfaces or utensils are unclean. Observed black build up and specs in the upper portion of the self service drink ice machine. PIC stated she would clean the ice machine that same day. Increase the cleaning frequency and/or detail of the ice machine.

3717-1-02.4(C)(17) / PIC: duties - ensure the facility has written procedures for responding to vomiting and diarrheal events.
Repeat No written procedures for responding to vomiting or diarrheal events.

3717-1-02.2(G) / Jewelry - prohibition.
Repeat Food employees wearing jewelry on arms or hands during food preparation. Observed employees working with food while wearing watches and bracelets. (G) Jewelry - prohibition. (1) Except as provided in paragraphs (G)(2) and (G)(3) of this rule, while preparing food, food employees may not wear jewelry on their arms or hands. (2) A medical alert bracelet is permitted when a reasonable accommodation is made, such as wearing the bracelet high on the arm or secured in a manner that does not pose a risk to the food but provides emergency medical information if it is needed. (3) A plain ring such as a wedding band may be worn.

3717-1-02.3(C) / Hair restraints - effectiveness.
Repeat Corrected During Inspection Food employee(s) not wearing a hair restraint. Observed an employee working without a hat or hair net. Visors are not approved as they have to cover the top of the head.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Nonfood-contact surfaces not easily cleanable. Observed rust forming on the racks in the walk in cooler. Remove the rust and reseal the racks. The rust makes the rack no longer smooth and easily cleanable.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Non-food contact surfaces of equipment are unclean. Observed food build up on the walk in cooler racks.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Facility not maintained clean. Observed the walk in cooler and walk in freezer floors in need of cleaning. Observed dusting needed around the air returns. Observed syrup needing cleaned off the floor by the soda syrup box stand. Observed cleaning needed behind the self service soda merchandising display cooler.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed sealant/grout missing from between some of the cove base molding. Observed portions of the drop ceiling coming apart. Observed the men's restroom door handle coming loose.

Due to excessive repeat violations, a reinspection shall occur on 4/30/2024, at which time all violations shall be corrected and/or reasonable timelines for correction provided.

Failure to do so may result in a summons to the Toledo Lucas County Health Department to explain the facility's lack of compliance.
