[?] A summary of the violations found during the inspection are listed below.

3717-1-03.3(H) / Reheating for hot holding.
Critical Corrected During Inspection Improper reheating of food for hot holding. Observed the refried beans and rice initially improperly reheated for hot holding. PIC had the items reheated to 165 degrees for at least fifteen seconds prior to placing them back into hot holding.

3717-1-03.4(F)(1)(a) / Time/temperature controlled for safety food - hot holding.
Critical TCS foods not being hot held at the proper temperature. Observed refried beans and rice holding at 128 degrees. PIC had the items properly reheated, then placed them back into hot holding.

3717-1-05.1(D) / Backflow prevention - air gap.
Critical Insufficient air gap between the flood rim and the water supply inlet. Observed air gaps needed at the server soda station ice bin and the bar ice bins. Install air gaps for backflow prevention.

3717-1-06.1(M) / Outer openings - protected.
Outer opening not protected. Observed the back door propped open at time of inspection. Once properly closed, observed a gap at the back door that could allow pests to gain entry to the facility. Ensure the back door remains closed at all times, unless receiving a delivery. When closed, ensure all gaps are eliminated to prevent pests from entering the facility.

Raw fruit and/or vegetables not washed or washed improperly. Observed avocados in the prep top table with stickers still on them, indicating they were not properly washed. Wash all produce prior to placing it in the prep top table.

3717-1-04.1(A) / Equipment and utensils - durability and strength.
Non-durable equipment observed. Observed the bottom shelf in the walk in freezer being broken and in need of repair.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Non-food contact surfaces of equipment are unclean. Observed the walk in cooler racks in need of being cleaned.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed the threshold to the walk in cooler needing repaired. Observed multiple burnt out light bulbs in the facility, needing replaced. Observed the ceiling tile over the make line tables needing placed properly in the ceiling, as it was loose.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Observed the ceiling tiles and electrical cords in need of dusting and cleaning.
