3717-1-02.4(A)(1) / PIC: Assignment of Responsibility
Critical No person in charge present in food facility during inspection. Manager reported at the end of the inspection. There must be a person in charge present at all times of operation.

3717-1-02.2 (C ) / Hands and arms: when to wash
Critical Food employee(s) did not wash hands when required. Observed an employee put on single use gloves without washing his hands first.

3717-1-03.2(A)(2) / Preventing contamination from hands - bare hand contact
Critical Improper bare-hand contact with RTE foods. Observed an employee cutting and handling tomatoes with no gloves on.

3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Food not properly protected from contamination by separation, packaging, and segregation. Observed raw chorizo stored above produce items in the walk in cooler. Observed sliced cheese stored in the same container as ground beef in the bottom of one of the prep top coolers. Cheese must be discarded.

3717-1-03.4(H)(1) / Ready-to-eat, time/temperature controlled for safety food - disposition.
Critical Ready-to-eat, TCS food not properly discarded when required. Observed a sauce in the walk in cooler dated the 28th. This is beyone the maximum 7 day hold time for refrigerated, ready to eat, TCS foods.

3717-1-02.3(A) / Food contamination prevention - eating, drinking, or using tobacco.
Employee eating, drinking, or using tobacco in non-designated area. Observed an employee beverage stored directly above the prep top cooler in the kitchen. All employee beverages must be stored separate from or below all food and clean food contact items.

3717-1-03.2(Y) / Miscellaneous sources of contamination.
Food not protected from contamination. Observed a squeeze bottle of sour cream stored directly in a container of cut lettuce in the prep top cooler. Do not store items directly in food to prevent contamination of the food. Observed a scoop stored with the handle buried in the lettuce in the walk in cooler.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Improper storage of food items. Observed onions on the floor in dry storage. Observed foods on the floor of the walk in freezer. Observed food items stored uncovered in the walk in freezer. All foods must be stored 6 inches up off the floor. All foods must be covered during storage to prevent contamination.

3717-1-05(H) / Capacity.
Water source and/or hot water capacity not sufficient.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired. Observed a leak at the kitchen hand sink drain.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Unnecessary or nonfunctional items and /or litter on premises. There are many nonfunctional items and pieces of unused equipment in the basement kitchen.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed low grout in the dish machine area, allowing water to pool.
