3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Repeat Corrected During Inspection Equipment food-contact surfaces or utensils are unclean. Observed ice wands in the walk in freezer without plastic wrap to protect them from contamination from the shelves. PIC had the wands cleaned and sanitized, then wrapped and placed back in the walk in freezer.

3717-1-05.1(D) / Backflow prevention - air gap.
Critical Insufficient air gap between the flood rim and the water supply inlet. Observed air gaps needed at the ice bins.

3717-1-03.2(Y) / Miscellaneous sources of contamination.
Corrected During Inspection Food not protected from contamination. Observed a pan of tamales, uncovered, in the walk in freezer. PIC had the tamales covered to prevent accidental contamination.

3717-1-04.4(B) / Cutting surfaces.
Cutting blocks or boards cannot be effectively cleaned and sanitized. Observed cutting boards having excessive staining/scarring in need of replacement or resurfacing.
