3717-1-04.4(N)(3) / Manual and mechanical warewashing equipment, chemical sanitization (quaternary ammonium) - temp., pH, concentration, and hardness
Critical Repeat Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness. Observed quaternary ammonium sanitizer concentration was over 400ppm. Quat sanitizer concentration shall have a concentration of between 150-300ppm for safety, as higher concentrations can be toxic if ingested from food contact surfaces. Corrective Action: Contact the chemical provider and have them adjust the levels so sanitizer is dispensed at the correct concentration

3717-1-04.4(S)(1) / Single-service articles and single-use articles - use limitation.
Re-use of single-service or single-use articles. Observed single use plastic spice container being re-used to store "house seasoning." Once the spice containers are empty of their original contents, they shall be discarded as they are not designed to be washed and re-used.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Equipment components are not intact, tight or properly adjusted. Observed torn gaskets on the small counter top cooler in the allergen station and on the cooler containing water in the Market. Replace torn gaskets to ensure coolers are holding at the proper temperature.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed ice/frost build-up inside the back walk-in freezer. Observed ice/frost build-up inside the small countertop cooler in the allergen station Observed build-up on hood filters in the kitchen. Clean non-food contact surfaces so they are free of build-up
