3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Repeat Observed food that was not properly protected from contamination by separation, packaging, and segregation. Observed eggs stored above ready to eat foods in prep table bottom section.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule.
3717-1-03.4(F)(1) / Time/temperature controlled for safety food - hot and cold holding.
Critical TCS foods were not being held at the proper temperature. Observed hot holding tamalies, chicken at 127 F by probing thermometer. Owner will either reheat to 165 F for 15 seconds (since food was out of cooking for apx. 20 minutes) or voluntarily pitch out.
To prevent the growth of pathogens, except during preparation, cooking, or cooling, or when time is used as a public health control, TCS food shall be held at 135°F or above (except that roasts cooked using time and temperature parameters in 3717-1-03.3 may be held at 130° F), or at 41°F or less.
3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Repeat Equipment food-contact surfaces or utensils are dirty. Observed dirty ice maker. Ice Maker scoop is also touching a paperboard roller from paper towels.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch.

3717-1-06(A)(1) / Indoor areas - surface characteristics
Observed floors, walls, and/or ceilings that were not constructed of approved materials. Observed need to repair surfaces (floors, walls, ceiling including cross section for back wall exhaust fan).
Where FSO or RFE activities are conducted, materials for indoor floor, wall, and ceiling surfaces shall be smooth, durable, and easily cleanable; closely woven and easily cleanable carpet for carpeted areas; and nonabsorbent for areas subject to moisture.
3717-1-06.1(M) / Outer openings - protected.
Repeat Observed an outer opening, window, or door without proper protection against the entry of insects and rodents. Observed need to remove fly catching ribbon from kitchen areas, keep screen doors in better repair to prevent infestations.
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule.

Measured 50 ppm chlorine in wipe cloth bucket. Temps are good throughout operation except where noted above: 41 F or less for coldholding/storages, frozen at 10 F or less and want 135 F or higher at all times (preheat equipment and check with probing thermometer).
