3717-1-06.2(B) / Handwashing cleanser - availability.
Critical Repeat Observed no supply of hand cleaning liquid, powder or bar soap at the handwashing sink(s). Two out of the three kitchen hand-sinks were not stocked with hand-soap.
To ensure proper cleaning of hands, each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap.
3717-1-06.2(C) / Handwashing sinks - hand drying provision.
Critical Observed no towels or hand drying device at the handwashing sink in the back kitchen area..
To prevent contamination by hands, each handwashing sink or group of adjacent handwashing sinks shall beprovided with: Individual, disposable towels; a continuous towel system that supplies the user with a clean towel; a heated-air hand drying device; or a hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures.
3717-1-07(B) / Working containers - common name.
Critical Observed working containers of poisonous or toxic chemicals that were not properly labeled with the name of the material. Outside salt being stored in a container without a label. For proper identification - label this container properly.
To prevent health hazards, working containers used for poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material.
3717-1-07.1(J) / Restricted use pesticides - criteria.
Critical Observed an unapproved restricted use pesticide being used Observed a household insect spray in a cabinet - do not apply this inside the facility. All pesticides must be applied by a licensed pest controller. Remove can from facility.
To prevent contamination, restricted use pesticides shall meet the requirements specified in 40 C.F.R. 152 Subpart I and be applied as specified under paragraph (C)(3) of this rule.

3717-1-03.2(D) / Food storage containers - identified with common name of food.
Food storage containers are not properly labeled. Observed a bulk bin of sugar being stored in the cabinet in the dining room without a label.
Working containers holding food or food ingredients that are removed from their original packages shall be identified with the common name of the food, except containers holding food that are readily and unmistakably recognized.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Observed the undercounter cooler with damaged/torn gasket - falling off the door. Make necessary repairs/replace the gasket and maintain.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed a spill of sticky substance on the container of salt in the cabinet by the drive through window. Properly clean this to prevent pest attraction.
Nonfood-contact surfaces of equipment shall be kept clean.
3717-1-06.4(A) / Repairing.
The physical facilities are not being maintained in good repair - observed several stained ceiling tiles in the chemical room. Replace tiles and maintain.

NOTES:
Observed very low flow at one of the hand-sinks. Try cleaning out the screen to increase flow of water. If this doesn't work, repairs may need to take place to achieve proper water flow for hand-washing.
Remember to use the hooks in the chemical room to hang wet mops to dry properly.
Remember to clean and sanitize the inside surfaces of the ice-machine regularly.
When storing personal drinks in the kitchen - make sure you store them underneath food prep areas or completely away from these areas to prevent contamination.
It is recommended to replace the carpet cove base molding by the back door with a material that is not absorbent and easily cleanable to prevent pest attraction/harborage and allow for easy cleaning.
