3717-1-05.1(O)(1) / Using a handwashing sink - accessible at all times
Critical Corrected During Inspection Handwashing sink near the register not accessible - blocked from use, items stored inside basin. Items were moved to access this sink.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Repeat Observed build-up on the inside surface of the ice-machine. Clean and sanitize surfaces - catch plate.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Repeat Corrected During Inspection TCS foods not being cold held at the proper temperature. Observed meats such as sausage and deli meat sitting out on counter - 55F. This food must be discarded. Do not let food sit out, place back in cooler once finished prepping with it.

3717-1-02.4(A)(2) / PIC - Manager Certification
Repeat Facility does not have an employee with manager certification in food protection assigned spefically to this store. At this time, only one person has a valid Level Two Manager Food Safety Certificate - Christopher Mowers, which covers all stores. This issue has been discussed on the previous SEVEN inpsections at this facility. One person per licensed facility must have a Level Two Food Safety Certificate.

Hand-sink dispenser broken - fallen off wall and sitting on its side next to sink, not useable.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed build-up on the front counter cooler gaskets.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Observed several places on the three compartment plumbing where there are continious drips of water.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed build-up on the floor and drain in the mop sink room. Observed build-up along the wall where the mop is being stored. Repair hooks to properly hang mop.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed ceiling area in front of walk-in cooler that is damaged, peeling paint, and evidence of moisture.

Store manager present during inspection - Kimberly Huber.
Masks - masks are required to be worn by all employees unless someone fits the exemption. If there is a medical exemption a written statement by a dr is required and maintain this documentation in personnel file. Employees who can not where a mask due to an exemption should be assigned tasks that limit interaction with other people.
Hair - hair on food and beverage handlers must be restrained by a covering such as hairnet or hat.
Test strips - make sure kits are opened and being used to determine concentration of sanitizer.

Some key items for operating now include
1. All employees must wear masks unless they fit under one of the exemptions
2. 6 foot separation or barriers in place
3. Written floor plan
4. Updated employee health log
5. Updated Covid19 training, available for free on line, 15 min video
6. Post signs of Covid19 symptoms in conspicuous place
7. Employee health assessments
8. Sanitizer in common areas
9. Limit people and post number, monitor
10. Cleaning commonly touched surfaces at least every two hours
