3717-1-02.4(A)(2) / PIC - Level Two Certified Manager
Repeat Facility does not have an employee with level two certification in food protection. At this time, only one person has a valid Level Two Food Safety Certificate - Christopher Mowers. This issue has been discussed on the previous four inpsections at this facility. One person per licensed facility must have a Level Two Food Safety Certificate.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Repeat Plumbing system not properly maintained or repaired. Observed a leak underneath the three compartment sink, dripping into bucket, gnats observed in this area. Observed constant drips coming from faucet. This is a repeat issue and needs immediate attention.

3717-1-2.3(C) Hair restraints - effectiveness.
Food employees shall effectively restrain hair by wearing hair restraints such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and worn to effectively keep their hair from contacting exposed food; clean equipment, utensils, or linens; or unwrapped single-service or single-use articles. This provision does not apply to food employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food; clean equipment, utensils, and linens; and unwrapped single-service and single-use articles.
