3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Repeat Equipment food-contact surfaces or utensils are unclean. Observed food accumultion on the cappuccino machine components. Thoroughly clean and sanitize the inner components of the cappuccino machine.

3717-1-02.4(A)(2) / PIC - Manager Certification
Repeat Facility does not have an employee with manager certification in food protection.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed the cabinets in the food service area all require deep cleaning. The cabinet below the fountain beverage and slushie machine has considerable liquid pooling in it. Thoroughly clean those cabinets to remove the liquid and make any necessary repairs to prevent further liquid from pooling.

Non-food contact surfaces of equipment are unclean. Observed spillage on the counter where the slushie machine and fountain beverage are located.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired. Observed a leak at the cold water knob on the hand sink in the women's restroom.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Facility not maintained clean. Observed build up, debris, dust on the floor, walls and ceiling of the facility. Thoroughly deep clean all physical surfaces.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed the grease trap lid is rusty and no longer easily cleanable. Replace the lid so that it fits tightly and does not require silicone sealing.
