Critical Repeat Corrected During Inspection TCS foods not being cold held at the proper temperature. Observed the mayonnaise and chipotle sauce holding at 46 degrees. Discussed cold holding options and techniques for lowering the temperatures of the squeeze bottles.

3717-1-05.1(D) / Backflow prevention - air gap.
Critical Insufficient air gap between the flood rim and the water supply inlet. Observed an air gap needed at the ice bin.

Repeat Facility does not have an employee with manager certification in food protection. ODH manager certification needed.

Repeat Irreversible registering temperature indicator not readily accessible for hot water mechanical warewashing operations.

Non-food contact surfaces of equipment are unclean. Observed the sandwich case needing wiped out.

Plumbing system not properly maintained or repaired. Observed multiple leaks at the three compartment sink. Observed a leak by the spray sink by warewashing.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Facility not maintained clean. Observed cleaning needed in hard to reach places throughout the facility.

Physical facilities not maintained in good repair. Observed cove base molding needing repaired in ware washing and by the walk in cooler door. Observed seals and caulking needing repaired/replaced by the walk in cooler door.
