[?] A summary of the violations found during the inspection are listed below.

3717-1-06.2(C) / Handwashing sinks - hand drying provision.
Repeat No towels or drying device at the handwashing sink(s). Provide paper towels at all handwashing sinks to allow for proper handwashing at all times. Observed that handwashing sinks were not stocked with paper towels at the time of the inspection.

3717-1-06.2(B) / Handwashing cleanser - availability.
Repeat No soap at handwashing sink(s). Observed that none of the handwashing sinks in the facility were stocked or supplied with soap. Provide handwashing soap at all handsinks at all times. Provide handwashing soap in both of the rest-rooms and at the handsink in the back stock room area.

3717-1-03.5(A) / Standards of identity
Packaged food labels are not in compliance with standards of identity. Observed the facility selling frozen pizzas that did not have any labels on them at all. Cease selling these pizzas as they are not labeled for individual sale. Pizzas were removed from sale.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed that the 3 door Pepsi cooler was not functioning or holding temperature. Repair or replace the Pepsi cooler so that it functions correctly.

3717-1-04(I) / Nonfood-contact surfaces - materials.
Nonfood-contact surfaces constructed of unapproved materials. Observed the facility lining the floor and the shelves of the walk-in cooler with cardboard. Remove all cardboard from the floor and the shelves of the walk-in cooler. Cardboard is absorbent and not cleanable. Cardboard is to be discarded once it is empty of it's original contents.

Plumbing system not properly maintained or repaired. Observed that the men's rest-room is filthy and the toilet tank top is missing. Replace the toilet tank top and clean the fixtures..

3717-1-06.4(H) / Cleaning and maintenance of plumbing fixtures.
Repeat Plumbing fixtures unclean. Observed that the men's rest-room is filthy. Thoroughly clean the men's rest-room and then maintain it clean. Clean all of the fixtures as well.

3717-1-06(A)(1) / Indoor areas - surface characteristics
Floors, walls, and/or ceilings not constructed of approved materials. Observed bare wood shelves in the walk-in cooler. Paint/seal all of the bare wood in the walk-in cooler. All surfaces must be sealed so they are non-absorbent.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed plaster/mudding work done behind the handwashing sinks in both of the rest-rooms. Finish the repairs and then paint the walls so that they are smooth and easily cleanable and non-absorbent.

Repeat Physical facilities not maintained in good repair. Observed missing and broken floor tiles in the facility. Replace all missing and broken floor tiles throughout.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Facility not maintained clean. Observed pop sprayed all over the ceiling and the wall. Thoroughly clean the facility and then maintain it clean.

3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
Repeat Floor and wall junctures not properly coved or closed and/or floor drains not provided. Replace all of the missing coving in the men's rest-room.

3717-1-06.2(I)(1) / Lighting - intensity (10 FC)
Light intensity less than ten foot candles in required areas. Replace all burnt out non-functional lights in the back stock area. Back stock area must have a minimum of 10 foot candles of light. Provide 10 foot candles of light in all areas of the back stock room.

901:3-4-16(C) / Level one Certification in Food Protection
Repeat RFE does not comply with Level One Certification requirements. Provide an employee on-site during all hours of operation that is food safety certified at Person In Charge Ohio level. Left handouts.
