No soap at handwashing sink(s).

Handwashing sign(s) not posted.

3717-1-06.2(C) / Handwashing sinks - hand drying provision.
No towels or drying device at the handwashing sink(s).

3717-1-03.5(C)(3) / Food labels - bulk food
Bulk food for customer self-service not properly labeled. Observed donuts in a display case that were not labeled and did not have any ingredient information.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Nonfood-contact surfaces not easily cleanable. Observed a coffee bar that was built and is not smooth or easily cleanable. Observed that all of the shelving in the walk-in cooler is not sealed and is not smooth and easily cleanable. Observed that the shelving throughout the facility is rusted and is not smooth and easily cleanable.

3717-1-04.3(B) / Fixed equipment installation - spacing or sealing.
Fixed equipment not properly sealed or spaced for cleaning. Observed that the three bay sink in the back room is not sealed to the wall properly.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Equipment components are not intact, tight or properly adjusted. Observed that gaskets are still torn on the doors of the walk-in cooler.

3717-1-04.2(J) / Cleaning and Sanitizers - Availability
Cleaning agents and/or sanitizers not available in the FSO or RFE. Correction: 1) Cleaning agents that are used to clean equipment and utensils as specified under rule 3717-1-04.5 of the Administrative Code, shall be provided and available for use during all hours of operation. (2) Except for those that are generated on-site at the time of use, chemical sanitizers that are used to sanitize equipment and utensils as specified under rule 3717-1-04.6 of the Administrative Code, shall be provided and available for use during all hours of operation.

No sanitizer test kit available.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Non-food contact surfaces of equipment are unclean. Observed buildup and debris all over in the back room during the inspection. PIC stated the debris is from the roof, the debris needs to be cleaned from the shelving in the back room.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed that the flooring throughout the facility needs to be cleaned.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed holes in the back room where the three bay sink is located. Observed that the handle on the walk-in cooler needs to be replaced.

901:3-4-07.0 / Facility layout and equipment specifications for RFE
RFE constructed, altered, and/or equipment added without prior approval from the licensor. Observed that the owner has added an unapproved coffee bar to the facility. The owner sells donuts, coffee, and cappuccino. Correction: No person, firm, association, organization, corporation, or government operation shall construct, install, provide, equip, or extensively alter a retail food establishment until the facility layout and equipment specifications therefore have been submitted to and approved in writing by the licensor. When the facility layout and equipment specifications are submitted to the licensor, they shall be acted upon within thirty calendar days after date of receipt. The facility layout and equipment specifications shall be approved by the licensor in accordance with the rules adopted pursuant to section 3717.05 of the Revised Code stating the criteria for facility layout and equipment specifications. Complaint on 19-Apr-2023 :
1. The hand sink located in the back room will need to be set up properly: soap, paper towels, and a garbage can.
2. A vomit/ fecal form will be sent through email as well as employee health agreements, read the documents and sign them if needed.
3. Corrections are listed behind each violation so that the violation can be corrected for the next visit.

4. No person, firm, association, organization, corporation, or government operation shall construct, install, provide, equip, or extensively alter a retail food establishment until the facility layout and equipment specifications therefore have been submitted to and approved in writing by the licensor. When the facility layout and equipment specifications are submitted to the licensor, they shall be acted upon within thirty calendar days after date of receipt. The facility layout and equipment specifications shall be approved by the licensor in accordance with the rules adopted pursuant to section 3717.05 of the Revised Code stating the criteria for facility layout and equipment specifications.

Discussed with Sam that plans will need to be turned in for the coffee bar, with a scaled drawing. Discussed items that will need to be corrected before an approval is given for the city license.
