[?] A summary of the violations found during the inspection are listed below.

3717-1-04.4(N)(3) / Manual and mechanical warewashing equipment, chemical sanitization (quaternary ammonium) - temp., pH, concentration, and hardness
Critical Corrected During Inspection Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness. Observed the mechanical quaternary ammonium dispenser dispensing above the required 200 to 400 ppm. PIC dumped out half of the sanitizer and filled the bucket the rest of the way with water so the concentration was 300 ppm.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are unclean. Observed black and brown build up in the ice machine. Increase the cleaning frequency of the ice machine.

3717-1-03.3(H) / Reheating for hot holding.
Critical Improper reheating of food for hot holding. Observed the marinara and meatballs improperly reheated in hot holding, as they were holding at 110 degrees. PIC was told to reheat the marinara and meatballs to 165 degrees prior to placing it back into hot holding.

3717-1-03.4(F)(1)(a) / Time/temperature controlled for safety food - hot holding.
Critical TCS foods not being hot held at the proper temperature. Observed the marinara and meatballs hot holding at 110 degrees. PIC was told to reheat the marinara and meatballs to 165 before returning them to hot holding.

3717-1-05.1(D) / Backflow prevention - air gap.
Critical Insufficient air gap between the flood rim and the water supply inlet. Observed no air gap at the prep sink nor at the customer self service soda machine and accompanying ice machine. Install air gaps for backflow prevention. (D) Backflow prevention - air gap. An air gap between the water supply inlet and the flood level rim of the plumbing fixture, equipment, or nonfood equipment shall be at least twice the diameter of the water supply inlet and may not be less than one inch (twenty-five millimeters).

3717-1-06.2(C) / Handwashing sinks - hand drying provision.
No towels or drying device at the handwashing sink(s). Observed the paper towel dispenser in the women's restroom not working. PIC stated it needed new batteries.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed the walk in cooler racks in need of cleaning due to food debris build up. Observed food debris on the drying racks. Increase the cleaning frequency of the racks in the facility.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired. Observed the three compartment sink's sprayer faucet leaking.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed the facility wall corner near the prep sink needing repair.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Observed the exterior and inside of the cabinets under the self service soda machine in need of cleaning, having excessive syrup and sticky surfaces. Increase the cleaning frequency of this area so it is maintained clean.

3701-21-25(I) / Person in charge certification in food protection
FSO did not have a person in charge per shift with the person in charge certification in food protection. When the person with Manager training in food protection is not present, an employee with Person in Charge certification needs to be present.
