3717-1-07.1(C) / Poisonous or toxic materials: Conditions of use.
Critical Observed unapproved application or use of a poisonous or toxic substance - hotshot ant and roach spray. All pesticides must be applied by a licensed pest control operator.
To prevent health hazards, poisonous or toxic materials shall be used and applied according to law and as specified in this rule.

3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Observed the hand-soap dispenser fallen off the wall where it was intended to be mounted. Last inspection it was noted to properly re-mount dispenser. You must use dispenser how it was designed to be used. If you wish to no longer use it - remove it and replace with another means of hand-soap. A stand alone pump soap is acceptable.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Repeat Observed the ice scoop cracked. Damaged utensils are no longer considered easily cleanable because pathogens can harbor inside cracks - remove from use.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.8(E) / Equipment, utensils, linens, single-service articles, and single-use articles - storage.
Repeat Observed cleaned coffee pots stored upright and directly next to hand-sink under the paper towel dispenser. To prevent contamination of food, food-contact surfaces, equipment, and utensils, clean equipment and utensils shall be stored in a self-draining position that allows air drying and covered or inverted. Do not store pots in this area because they could become contamination when washing hands.
Cleaned equipment and utensils, laundered linens, single-service articles, and single-use articles shall be properly stored as described in this rule.
3717-1-05(I) / Pressure.
Repeat Water under pressure is not available to all fixtures and equipment that are required to use water. The hot water does not flow at sufficient velocity to properly wash hands - hand-sink closest to dining register in front.
To ensure proper cleaning and sanitation, water under pressure shall be provided to all fixtures, equipment, and nonfood equipment that are required to use water.
3717-1-06.4(F) / Drying mops.
Repeat Observed mops being dried using an unapproved method. Do not sit wet mop heads on the floor - this does not allow for proper drying, could attract pests, and could damage flooring.
After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies.
