[?] A summary of the violations found during the inspection are listed below.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical TCS foods not being cold held at the proper temperature. The ranch dressing in the True sliding door cooler was 45F. All TCS food shall be held at 41F or less to prevent the growth of bacteria. Adjust the cooler colder.

3717-1-06.4(K) / Controlling pests.
Critical Presence of live insects, rodents, and other pests. Observed a couple flies in the facility. Flies can be a vector of disease and shall be prohibited from the facility. Eliminate the pest and contact a licensed PCO if needed.

3717-1-06.1(K) / Insect control devices - design and installation.
Improper use or placement of insect control devices. Observed fly paper hanging over areas where food could be contaminated. Relocate the fly paper to a space where there is no chance of contamination to food, food prep areas, equipment and utesils and single-use articles.

3717-1-04.4(S)(1) / Single-service articles and single-use articles - use limitation.
Repeat Re-use of single-service or single-use articles. Observed old food containers being re-used to store different food. Provide reusable food containers for the storage of food and eliminate the single-use food containers from the facility.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Observed build up on the hood filters and some grease on the floor under the cookline.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed two missing ceiling vents and some missing floor tiles.
