Critical Food not properly protected from contamination by separation, packaging, and segregation. Observed raw Bacon and Eggs sitting above RTE foods, in the Dasani cooler. Correction: RTE/ TCS foods shall be separated to prevent contamination.

Improper storage of food items. Observed drinks throughout the store, sitting directly on the floor. Discussed with the owner that all food items need to be off the floor.
3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Equipment components are not intact, tight or properly adjusted. Observed that the gaskets on the walk in cooler are torn and need to be replaced.
3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed that the handle on the walk-in cooler door is loose and will need to be repaired.
Observed that there is a hole in the wall in the back room near the three bay sink.
Observed that some of the ceiling tiles on the sales floor need to be repaired.

| 1. The owner was available for the inspection. 2. The owner fixed the plumbing on the three bay sink. 3. The facility got rid of the coffee pot but there was still coffee supplies at the facility. The facility is only a risk level I which is not under risk level I activities. |
