[?] A summary of the violations found during the inspection are listed below.

3717-1-05.1(O)(1) / Using a handwashing sink - accessible at all times
Critical Repeat Handwashing sink not accessible. Observed the handwashing sink next to the 3 compartment sink was not accessible and was blocked from being used. Corrective Action: Provide free access to this sink at all times.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are unclean. Observed the facility re-using tin cans for utensil storage. Provide multi use containers that are smooth and easily cleanable for storing utensils in. Once a tin can is empty of it's original contents it must be discarded. Cans are considered single use and thus cannot be washed or reused. Corrective Action: Facility removed the tin cans during the inspection.

3717-1-03.4(F)(1)(a) / Time/temperature controlled for safety food - hot holding.
Critical Repeat TCS foods not being hot held at the proper temperature. Observed TCS foods in the steam table at 96 degrees internally. Steam tables cannot be used to heat or reheat foods. Foods must be cooked or reheated to required internal temperatures and then placed in the pre-warmed steam table where they must maintain at least 135 degrees internally. Corrective Action: All foods in the steam table that were not at temperature were placed into the oven to be heated to at least 165 degrees prior to placing them back in the steam table. Note: Facility is not licensed to cool and reheat in bulk. All cooling and reheating in bulk is to cease.

3717-1-07.2 / Poisonous or toxic materials: storage and display - separation
Critical Improper display of poisonous or toxic materials offered for retail sale. Observed single service items stored below chemicals on the isle shelves. Store all single service items above any and all chemicals to protect them from contamination. Corrective Action: Move the single service items so that they are no longer located below chemicals on the shelves.

3717-1-05.1(D) / Backflow prevention - air gap.
Critical Insufficient air gap between the flood rim and the water supply inlet. Observed that someone replaced the drain on the food prep sink and removed the required air gap on the drain. Corrective Action: Have a licensed plumber come out and re-plumb the food prep sink and reinstall the required air gap on the drain line. All plumbing must be done to the Ohio Plumbing Code and must be done by a licensed plumber with all required permits pulled.

3717-1-06.2(C) / Handwashing sinks - hand drying provision.
Repeat No towels or drying device at the handwashing sink(s). Observed that the handsink next to the 3 compartment sink had no paper towel available. Provide a means to dry hands at the handwashing sink at all times.

3717-1-06.2(B) / Handwashing cleanser - availability.
Repeat No soap at handwashing sink(s). Observed no hand washing soap available at the handwashing sink next to the 3 compartment sink. Provide hand soap at all handwasing sinks at all times.

3717-1-03.5(A) / Standards of identity
Packaged food labels are not in compliance with standards of identity. Observed the facility offering for sale pop sickles and fruit cups that were not labeled for individual retail sale and thus had no label at all. All prepackaged foods must be properly labeled for individual retail sale.

3717-1-03.5(C)(2) / Food labels - packaged food
Label on a food packaged in-house did not contain all required information. Observed the facility offering individual cups of ice for retail sale. Cups of ice were not labeled at all. All foods packaged in-house must be labeled with: Store name, store address, contents, ingredients, and net weight. Provide labels on the cups of ice or cease offering them for sale.

3717-1-06.1(M) / Outer openings - protected.
Repeat Outer opening not protected. Light from the outside is visible under the front exterior door. Replace the door sweep on the front door. All exterior openings must seal tight to prevent the entrance of pests.

3717-1-03.2(R) / Food storage - prohibited areas.
Food stored in prohibited area. Observed foods stored below chemicals and toxic items on the shelves. Store all foods above any chemicals or toxic items to prevent contamination of the food products.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Repeat Improper storage of food items. Observed foods stored on the floor of the facility as the facility is overstocked. Foods must be stored at least 6" up off the floor to prevent contamination.

3717-1-03.2(K) / In-use utensils - between-use storage.
Repeat Corrected During Inspection In-use utensils improperly stored. Observed the in-use food serving utensils stored in standing water at room temperature. Serving utensils must be stored in hot water that is mechanically maintained at 135 degrees or greater at all times or in a clean and sanitary location and then cleaned and sanitized at least every 4 hours. Utensils were removed from the standing water and properly cleaned at the time of the inspection.

3717-1-04.8(E)(1) / Equipment, utensils, linens - storage.
Observed the facility reusing cardboard boxes for storage of other items. Cardboard boxes are to be discarded once empty of original contents. Cardboard cannot be washed and sanitized and thus cannot be reused for any purpose.

3717-1-04.4(S)(1) / Single-service articles and single-use articles - use limitation.
Re-use of single-service or single-use articles. Observed the facility washing and reusing single service tin pans. Single service tin pans can be used once and then must be discarded. All of the tin pans were discarded at the time of this inspection.

3717-1-04(I) / Nonfood-contact surfaces - materials.
Nonfood-contact surfaces constructed of unapproved materials. Observed cardboard wedged in under the shelves in the walk-in cooler. Remove the cardboard from the under the shelves and on the floor. Floors must be smooth and easily cleanable. Floor in the walk-in cooler is filthy.

3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Equipment not approved by a recognized testing agency. Observed that the facility added two homestyle chest freezers for storage and cup ice sales. The addition of the freezers was not approved and homestyle equipment is not allowed. Remove the chest freezers from the facility.

Repeat Equipment not approved by a recognized testing agency. Observed the facility using a home style fryer. Only commercial NSF approved equipment can be used in the facility. Remove all homestyle equipment from the facility. (If an additional commercial NSF fryer is added it will need to be approved by the health department and the fire department prior to installing it).

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed a condensate leak on the interior of the 1 door freezer in the main cooking area. Have the freezer repaired so that it no longer leaks condensate on the interior of the unit.

3717-1-04(I) / Nonfood-contact surfaces - materials.
Repeat Nonfood-contact surfaces constructed of unapproved materials. Observed bare wood shelves in the walk-in cooler. All wood must be sealed to be non-porous and be smooth and easily cleanable. Paint/seal all of the bare wood shelves and supports in the walk-in cooler.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Equipment components are not intact, tight or properly adjusted. Observed split/torn door gaskets on the walk-in cooler doors. Replace all split/torn door gaskets.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed that the hood is in need of cleaning. Hood sticker indicated that it was due to be cleaned on 6/23/2022. Hood is way past the cleaning date. Schedule a hood cleaning right away.

3717-1-05.4(P) / Maintaining refuse areas and enclosures.
Improperly cleaned storage area for refuse, recyclables, or returnables. Observed grease and debris around the grease dumpsters. Clean and power wash the area around he grease dumpsters. Maintain the area clean.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Observed an accumulation of dust and dirt all over the front windows and window sill area. Clean the front of the store and then maintain it clean.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Unnecessary or nonfunctional items and /or litter on premises. Observed homestyle and unnecessary equipment stored in the loft above the 3 compartment sink area. Remove all of the homestyle equipment and all of the unnecessary items from the loft area. This area is creating a pest harborage.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Facility not maintained clean. Observed the floor throughout the facility in need of cleaning including in the rest-room and the walk-in cooler. Thoroughly clean the floor and then maintain it clean.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed the walls around the mop sink with water damage and rot. Replace the walls around the mop sink as they are no longer in good repair.

Repeat Physical facilities not maintained in good repair. Observed acoustic ceiling tiles in the main kitchen area. Only vinyl faced ceiling tiles are permitted in this area as they must be smooth and cleanable. Replace the ceiling tiles in the main kitchen area with vinyl faced tiles. Note: several tiles in this area are water damaged and need replacing.

Repeat Floors, walls, and/or ceilings not smooth and easily cleanable. Observed water damaged ceiling tiles throughout the facility. Replace all water damaged ceiling tiles.

3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
Repeat Floor and wall junctures not properly coved or closed and/or floor drains not provided. Observed missing and loose coving throughout the facility. Replace all missing coving and reattach all loose coving.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed the walls with holes and damage throughout the facility and in the back stock room. Observed the wall behind the hand sink next to the 3 compartment sink in poor repair. Observed the door jamb area to the back stock room in poor repair. Repair the walls throughout so that they are smooth and easily cleanable and in good repair.

Repeat Physical facilities not maintained in good repair. Observed peg board installed as a wall in the kitchen area. Peg board is not smooth hand easily cleanable. Remove the peg board.

3717-1-06.2(I)(1) / Lighting - intensity (10 FC)
Repeat Light intensity less than ten foot candles in required areas. Observed 1.7 foot candles of light in the walk-in cooler, 10 foot candles are required. Provide additional lighting in the walk-in cooler.

901:3-4-07.0 / Facility layout and equipment specifications for RFE
RFE constructed, altered, and/or equipment added without prior approval from the licensor. Facility added multiple pieces of homestyle equipment to the facility. Facility is not to alter any equipment in the facility without notifying the health department first. Homestyle equipment is not allowed and the equipment is to be removed immediately.
