[?] A summary of the violations found during the inspection are listed below.

3717-1-02.4(C)(11) / PIC: duties - ensure employees are proplery sanitizing cleaned equipment and utensils
Critical PIC not ensuring employees are properly sanitizing equipment and utensils and monitoring of sanitizer parameters. At the time of this inspection facility was offering foods that required tongs and no one in the facility could explain proper warewashing. The facility also did not have sanitizer, sanitizer test strips, or an approved 3 compartment sink for warewashing. All sales of open foods that require any utensils are to stop. Corrective Action: All open foods requiring tongs were removed from sale.

3717-1-02.4(C)(15) / PIC: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical Employees are not informed in a verifiable manner of their responsibility to report information about their health. Corrective Action: Provide employee health reporting forms for all employees in the facility. These must be on file at all times in the facility.

3717-1-02.2 (A) / Hands and arms: clean condition.
Critical Food employee(s) hands and/or arms not clean. Facility does not have an accessible handsink that is plumbed to Ohio plumbing code. Employees have no way to properly wash their hands and arms. Corrective Action: Facility is to have the handsink moved and plumber according to Ohio Plumbing code by a licensed plumber.

3717-1-04.6(B) / Sanitizing frequency of utensils and food-contact surfaces - before use and after cleaning
Critical Utensils and food-contact surfaces of equipment not sanitized at the required frequency. Observed the facility using tongs to serve food. The facility does not have any way to properly sanitize the tongs after washing them. No one on site could explain how to properly wash, rinse, and then sanitize the tongs as is required by law. Corrective Action: Facility is to no longer offer an foods that require warewashing OR the facility must supply all necessary cleaners and sanitizers for the proper washing of the tongs. Employees must be trained in how to properly wash, rinse, and sanitize food contact equipment. Facility removed the products from sale at the time of the inspection.

3717-1-02.4(C)(17) / PIC: duties - ensure the facility has written procedures for responding to vomiting and diarrheal events.
No written procedures for responding to vomiting or diarrheal events.

3717-1-06.2(C) / Handwashing sinks - hand drying provision.
No towels or drying device at the handwashing sink(s). Observed that the front handsink and the handsink in the rest-room were not supplied with paper towels, thus employees have no way to properly wash and dry their hands. Provide paper towels for proper hand drying at both sinks at all times. Employees must be able to properly wash their hands to prevent the spread of foodborne illness.

3717-1-05.1(L) / Handwashing sinks - location and placement.
Handwashing sink(s) are not conveniently located as specified in rule. The handsink is to be moved so that it is no longer draining into the 3-compartment sink. The handsink is to be properly installed and plumbed to the Ohio Plumbing Code by a licensed plumber.

3717-1-03.5(A) / Standards of identity
Packaged food labels are not in compliance with standards of identity. Observed the facility selling sleeves of crackers, sleeves of cookies, single sticks of butter, Capri Sun individual pouches, and Hug individual drinks. These products are not labeled for individual sale and thus cannot be sold individually.

3717-1-03.5(C)(1) / Food labels - packaged food
A food packaged in the facility not labeled. Observed the facility selling cups of ice that are not labeled in any way. All cups of ice must be labeled with the store name, store address, Ice, and the net weight of the ice. Provide proper labels or stop selling the ice.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Improper storage of food items. Observed the facility storing boxes of candy on the floor behind the counter. All food must be stored at least 6 inches up off the floor to prevent possible contamination.

3717-1-03.2(K) / In-use utensils - between-use storage.
In-use utensils improperly stored. Observed in-use utensils that were stored in an unsanitary area and were not being properly washed, rinsed, and sanitized. Store tongs on a sanitized surface between uses then wash, rinse, and sanitize them at least once every 24 hours.

3717-1-04(I) / Nonfood-contact surfaces - materials.
Nonfood-contact surfaces constructed of unapproved materials. Observed cardboard on the shelves in the facility. Remove all cardboard from the shelves as it's not cleanable. Provide a cleanable surface on all shelves throughout the facility.

3717-1-04.1(KK) / Food equipment - certification and classification.
Equipment not approved by a recognized testing agency. Observed equipment in the facility that is no longer being used or no longer functions as it was approved. Remove all equipment that is not longer functioning or no longer being used for it's intended purpose. The 2-door cooler in customer area (being used for storage and does not function), must be removed from the facility.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed torn, split, and missing door gaskets on the doors to the walk-in cooler. Replace all damaged and missing gaskets. Do not use duct tape on the door gaskets. Thoroughly clean the door jambs.

3717-1-04.4(D) / Warewashing equipment - cleaning frequency.
Warewashing equipment and/or components not cleaned at required frequency. Observed the 3 compartment sink in a filthy state. Thoroughly clean the 3 compartment sink and maintain it clean. Handsink drain cannot drain into the 3 compartment sink, remove it immediately.

No sanitizer test kit available. Provide sanitizer and the correct sanitizer test kit at all times.

3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Non-food contact surface(s) not cleaned at the required frequency. Observed the floor of the walk-in cooler with a build-up of dirt and debris. Thoroughly clean the floor of the walk-in cooler. Clean all of the door jambs of the walk-in cooler doors. Maintain all equipment in the facility clean.

3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Non-food contact surface(s) not cleaned at the required frequency. Observed the bottom of the Home City ice freezer in need of cleaning. Thoroughly clean the inside of the Home City Ice freezer and then maintain it clean.

3717-1-05.1(B)(1) / Approved plumbing system.
Plumbing system does not comply with the Ohio plumbing code. The 3-compartment sink, the handsink, and the mop sink must be plumbed to Ohio Plumbing code. Hire a licensed plumber to properly plumb these fixtures.

3717-1-05.4(D) / Receptacles.
Improper receptacles for refuse, recyclables, or returnables. Observed the facility using cardboard boxes for customer trash cans. Provide washable proper trash cans in the facility. Outside trash cans must be covered to prevent the trash from blowing out and around.

3717-1-05.4(G) / Storage areas, rooms, and receptacles - capacity and availability.
Adequate area and/or receptacles for refuse, recyclables, or returnables not provided. Provide trash cans at all handsink locations for disposing of paper towels after proper handwashing. Supply a trash can in the rest-room and at the front handsink.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Clean the floor throughout the facility and maintain it clean on a daily basis.

Facility not maintained clean. Observed the facility in need of a deep cleaning. Clean the floors, shelves, equipment, and the ceiling vents throughout the facility.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Unnecessary or nonfunctional items and /or litter on premises. Observed the following items in the facility that are not necessary: old shelving, slicer, pizza oven, cooler, mini fridge, coffee pot. All unnecessary items and equipment are to be removed from the facility to allow for proper cleaning and organization of necessary items.

Facility not maintained clean. Thoroughly clean and organize the entire back room. Remove all items that are not necessary to the facility.

3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
Floor and wall junctures not properly coved or closed and/or floor drains not provided. Provide cove molding throughout the facility where it is missing.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed the wall around the mop sink with excessive damage. Repair the wall so that it is smooth and easily cleanable.

Unnecessary or nonfunctional items and /or litter on premises. Observed the ice freezer outside the front of the store no longer works. Have the ice freezer removed.
