3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed food debris in the bottom of the reach-in freezer. Thoroughly clean the freezer.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Non-food contact surfaces of equipment are unclean. Observed black build up in the cabinet below the front hand sink. Thoroughly clean the cabinet to remove all of the build up.

Repeat Non-food contact surfaces of equipment are unclean. The cabinet below the fountain beverage area is filthy with black build up and areas with liquid accumulating. Thoroughly clean the entire cabinet to remove all build up and eliminate the source of the moisture.

Repeat Non-food contact surfaces of equipment are unclean. Observed spillage on the fountain beverage counter top. Thoroughly clean the counterop including under equipment.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired. Observed a leak at the men's bathroom hand sink hot water knob. Repair it to eliminate the leak.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed the cabinets below the fountain beverage area are in extremely poor repair. Replace the cabinetry as it is no longer smooth, non-abosrbent or easily cleanable. This is a repeat violation and requires immediate attention.

Repeat Physical facilities not maintained in good repair. Observed the grease trap lid is extremely rusty and no longer tight fitting. Repair/replace the lid so that it is easily cleanable and fits tightly without the aid of a silicone seal.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Facility not maintained clean. Observed build up/dust on the floors, walls and ceiling of the facility to include the walk-in cooler floor and light fixtures. Thoroughly deep clean the physical surfaces.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed water damaged ceiling tiles in the facility. Replace the tiles accordingly.
