[?] A summary of the violations found during the inspection are listed below.

3717-1-04.4(N)(3) / Manual and mechanical warewashing equipment, chemical sanitization (quaternary ammonium) - temp., pH, concentration, and hardness
Critical Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness. Observed the quaternary ammonium sanitizer dispenser dispensing sanitizer below 100 ppm. PIC mixed a sink of quaternary ammonium sanitizing water manually and stated maintenance would be called to fix the dispenser.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Corrected During Inspection TCS foods not being cold held at the proper temperature. Observed food in the bottom of the make table holding at 44.5 to 46.5 degrees. PIC moved all of the food from the bottom of the make table into the walk in cooler.

3717-1-06.1(M) / Outer openings - protected.
Outer opening not protected. Observed the back door of the facility propped open, potentially allowing insects, rodents, and other pests into the facility. PIC closed the door.

3717-1-04.1(A) / Equipment and utensils - durability and strength.
Non-durable equipment observed.Observed the make table not holding lower food below 41 degrees. PIC moved all the food into the walk in cooler and said maintenance would be called to service the make table.

3717-1-04.2(J) / Cleaning and Sanitizers - Availability
Corrected During Inspection Cleaning agents and/or sanitizers not available in the FSO or RFE. Observed no sanitizing solution set up at time of inspection. Ensure a sanitizing bucket with a proper concentration of sanitizer, is set up prior to food prep/service.

3717-1-04.2(I) / Sanitizing solutions - testing devices.
No sanitizer test kit available.Observed expired quat sanitizer testing kits. PIC stated new kits would be purchased.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed the pizza oven vent hood not functioning. The lights were not working and hot air and smoke were not venting.

901:3-4-16(C) / Level one Certification in Food Protection
Repeat RFE does not comply with Level One Certification requirements.Obtain proper food protection training for employees. Level One, Person in Charge, training is currently free on ServSafe.
