[?] A summary of the violations found during the inspection are listed below.

3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Repeat Corrected During Inspection Food not properly protected from contamination by separation, packaging, and segregation. Observed raw shell eggs being stored above milk containers in the cooler. Store raw shell eggs on the bottom shelf of the cooler below any ready to eat/drink items to prevent cross contamination. Corrective Action: Eggs were moved to the bottom shelf.

3717-1-06.2(B) / Handwashing cleanser - availability.
No soap at handwashing sink(s). Provide hand soap in both rest-rooms at all times.

3717-1-06.2(C) / Handwashing sinks - hand drying provision.
No towels or drying device at the handwashing sink(s). Provide paper towels in both rest-rooms at all times.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed a build-up of dirt and dust on the doors and the door jambs of the walk-in cooler. Thoroughly clean and maintain the walk-in cooler and all of it's doors clean.

3717-1-06.4(H) / Cleaning and maintenance of plumbing fixtures.
Plumbing fixtures unclean. Thoroughly clean the men's rest-room handsink and toilet. Replace the missing tank cover on the toilet. Maintain the rest-room clean at all times.

3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
Floor and wall junctures not properly coved or closed and/or floor drains not provided. Observed all of the coving in the men's rest-room was removed. Replace the coving in the men's rest-room.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Observed pop that had exploded all over the walls, ceiling, and light fixture in the area in front of the coolers. Thoroughly clean the walls, floor, ceiling, and light fixture in this area. Be sure all of the sprayed pop is removed.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed water damaged ceiling tiles in the facility. Facility is currently in the process of replacing the water damaged ceiling tiles.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Observed the floor of the walk-in cooler with a build-up of dust and dirt. Thoroughly clean the floor of the walk-in cooler and maintain it in a clean state.

3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings not smooth and easily cleanable. Observed the wall behind the handsink in the women's rest-room was repaired and then never painted. Paint the wall in the women's rest-room so that it is smooth and easily cleanable.

901:3-4-16(C) / Level one Certification in Food Protection
RFE does not comply with Level One Certification requirements. Facility must have a person in charge at all times of operation that has level I food safety certification. Certificates must be posted and available at the facility. See attached handout for course offerings.

PIC at the time of the inspection today was Jeemit.

Note: The facility has a two compartment sink that it currently uses one bay for hand washing and the other as the mop sink. This is allowed as the facility does not offer any open food or beverages. The facility would be required to supply a 3 compartment sink, a hand sink, and a mop sink if fountain drinks, coffee, cappuccino, any open food or beverages were offered for sale, or ice bagged in house were to be offered. Any changes to equipment or foods offered for sale would have to be approved by the health department in advance.

Consulted: The wood on the shelves in the walk-in cooler needs to be sealed to prevent moisture damage and to also allow for cleaning.

Consulted: All employee food and belongings need to be stored in a separate location from food and drinks sold in the facility. Employee items are not to be stored above food and drinks sold in the facility. Manager stated that he would change the shelf organization around.

Observed folded blankets and a pillow in the facility. Remove these items from the facility.

Consulted: Contact Home City Ice and have the ice freezer serviced due to a frost build-up on the inside.

Note: Chest freezer was not in use at the time of this inspection.
