Corrective Action(s): Eye goggles for Laser Hair removal require a High Level Disinfectant as they are in contact with the mucous membrane of the clients -discussed at length her options here. Operator is to purchase a high level disinfectant today and disinfect ALL goggles. NOTE: ensure your comedone extractors for facials is high level disinfected here as well Violation Score: 15

Corrective Action(s): All Personal Service Establishments are required to have an Infection Control Plan for training and review purposes for proper cleaning and disinfection of equipment. Submit within 1 week for review Violation Score: 1 633 - Equipment / instruments / products not of suitable material and/or not maintained in good repair Observation (CORRECTED DURING INSPECTION): Laser Hair Removal treatment rooms all have monitors which is a reflective surface and is not covered during treatment. Corrective Action(s): All reflective surfaces must be covered during treatment like the mirrors and the blinds. Cover the monitors with a towel during treatment. Violation Score: 9

Services currently offered are Laser Hair Removal (body contouring), facials, Body Slimming. -Metricide sterilizer/high level disinfectant not recommended due to good ventilation is required -used for the comedone extractors for facials NOTE: operator to provide full names of Laser Devices used (Three Class 4 devices observed: Syneron, Fotona, Picosure) -all these devices must be Health Canada approved as per page 6 of the Ministry of Health Guidelines booklet provided -goggles for laser devices are match rated 755nm, adequate signage on outside of doors, blinds for mirrors when laser is in use -low level disinfectant Lysol wipes used is adequate -Checklist for Laser provided for review -all handsinks well stocked and clear for use NOTE: No lancets, tweezers for ingrown hairs, extractors needle and loops used for facials as per operator -this must be specified in your Infection Control Plan -Operator is not to provide any foods to customers. Only bottled water from disposable cups and drip coffee (powdered cream only) is allowable Follow-up Inspection next Friday
