3701-31-04 ( D )(6)(a) / ORP reading shall be 650 millivolts or greater.
Repeat ORP reading was below 650 mV.
Have the ACC unit inspected to determined why the ORP is low.
3701-31-04 (B)(4) / The licensee /authorized representative is keeping records
Repeat The licensee /authorized representative was not keeping or maintaining operational records on a daily basis as required. Records show that the pool was not tested for 4 straight days.
All pool chemistry tests must be conducted at code mandated frequencies with results recorded in facility records.
3701-31-04 (B)(4)(a)(i) / Disinfection residuals are tested daily at pool side at required intervals for pools/automatic chemical controllers/special features
Repeat Disinfection residuals were not tested daily at pool side and/or at the required intervals for pools/automatic chemical controllers/special features. Pool was not tested for four consecutive days.
Test the disinfection residual every day before bathers enter the water. Disinfection residual must then be tested every 4 hours when an ACC is not in use. If an ACC is in use the disinfectant residual must be tested every 12 hours while the pool is in use. For spraygrounds and spray features, the disinfectant residual must be tested at 1 spray feature every 6 hours the sprayground or spray feature is in use.

Depth markers were not maintained in a clearly legible/visible manner.
Replace the depth markers so that bathers are able to clearly see the depth of the water.
