[?] A summary of the violations found during the inspection are listed below.

3701-31-04 (B)(1)(c) / Minimum disinfection residual shall be maintained as required.
Critical Observed the free chlorine in the pool to only be 0.5 ppm.
Ensure at least 1 ppm free chlorine in the pool. Recommend 2-4 ppm.
3701-31-04(D)(6)(a)(2) / Pool free chlorine disinfectant residual shall be above 1 ppm.
Critical Pool free chlorine disinfectant residual was below 1 ppm. Observed only 0.5 ppm free chlorine in pool.
Do not operate the pool until the chlorine disinfectant residual is at 1 ppm or above.

3701-31-04 (A) / Approval shall be obtained for any construction or alterations of a public swimming pool.
Repeat Observed the main pool pump and motor to have been replaced from what was recorded on the Equipment Inventory Report from the previous operating year. Pump and motor are a PENTAIR CHALLENGER 346201. Pump replacement needs to be exact make and model. Pump motor replacement must be within 10% of the previous output.
Complete an Equipment Replacement Notification (ERN) form and submit it to the Ohio Department of Health for the new pump and motor.
3701-31-04 (B)(4) / The licensee /authorized representative is keeping records
Repeat The licensee/authorized representative was not keeping or maintaining operational records. Observed the operational records for the swimming pools to be incomplete. Water quality test results are not be taken and/or recorded on all days the pool is open to bathers nor at the required frequency.
All facility records must be maintained and available for review upon request.
3701-31-04 (B)(4)(a)(i) / Disinfection residuals are tested daily at pool side at required intervals for pools/automatic chemical controllers/special features
Repeat Disinfection residuals were not tested daily at pool side and/or at the required intervals for pools without an ACC.
Test the disinfection residual every day before bathers enter the water. Disinfection residual must then be tested every 4 hours when an ACC is not in use.
3701-31-04 (B)(4)(a)(iii) / pH is tested daily at pool side at required intervals for pools/automatic chemical controllers/special features
Repeat pH was not tested daily at pool side and/or at required intervals for pools without an ACC.
pH must be tested prior to bathers entering the water and every 4 hours the pool is open for use.
3701-31-04 (B)(4)(c) / SVRS/APSOS/ACC test results/ service records are maintained
Repeat No SVRS test results and/or service records were available at time of inspection.
The test results/service records for these devices must be kept on site for review. If the device has not been recently serviced, have the device tested immediately and provide documentation of the test results.
3701-31-04 (B)(6) / Equipment is maintained per manufacturers specifications.
Repeat Equipment was not maintained per manufacturers specifications. Observed the pool interior paint to be chipping and peeling in several locations.
All equipment must be maintained in accordance with the manufacturers specifications. If resurfacing or repainting the pool, the interior shall be white, an approved color or approved by the Director of Health.
3701-31-04 (B)(6)(p) / Deck surfaces are free of standing water or other hazards.
Repeat Deck surfaces had standing water or other hazards. Observed the pool deck to be deteriorating around the skimmer box on the south corner of the pool. Observed the pool deck corner broken off at the north side of the pool and deterioration/chipped cement east side of the pool in front of the wading pool.
Repair areas of the deck where standing water is occurring or sharp edges or other hazards are present. If more than 50% of the deck must be replaced, plans must be submitted to the Ohio Department of Health for review and approval.
3701-31-04 (D)(8)(c) / Test kit reagents fresh/ properly stored.
Repeat Test kit reagents were not fresh or stored properly. Observed several kit reagents to be expired.
Obtain fresh reagents and store them properly.
3701-31-04 (E)(2)(d) / First aid kit provided
Repeat First aid equipment was not provided. Observed the first aid kit to be located in the equipment and chemical storage shed. Observed first aid kit without the minimum supplies.
Ensure the first aid kit is located pool side and accessible to pool patrons. Ensure the first aid kit is supplied with a minimum of disposable gloves, gauze and bandages.

The following violation(s) have been corrected since the last inspection.
3701-31-04 (B)(6)(k)/Outlet drain covers are secure/meet ANSI/ASME A112.19.8-2007 requirements.
Observed the equalizer lines to be fully operational, uncovered and open. Equalizer lines shall have compliant covers, or be removed or be plugged. Covers shall meet the requirements of ANSI/ASME A112.19.8-2007 and be secure and unable to be removed without the use of tools.
Ensure the equalizer lines are covered, removed or plugged. Will follow-up.
Comments
--Free chorine in pool was found to be only 0.5 ppm. Recommend at least 2-4 ppm free chlorine. Ensure the minimum of 1 ppm free chlorine is maintained at all times. Pool was voluntarily closed at time of inspection. Recommend pool remain closed to bathers until free chlorine is maintained at or above 1 ppm.
--Observed the pool vacuum lines to be disabled from inside the pump room.
--Equalizer lines were plugged below the skimmer baskets at time of inspection.
