3701-31-04 (B)(6)(t) / Equipment/chemical/storage areas are free of hazards/accessible only to authorized personnel.
Critical The outside filters and pump are accessible to unauthorized personnel.
Secure the outside filters and pump so that only authorized personnel have access.

3701-31-04 (A) / All construction or alterations of a public swimming pool are approved
A new pump was installed and is not yet approved by ODH.
Submit the Equipment Replacement Notification Report to ODH for approval.
3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
The turnover rate was 8.8 hours. The turnover rate should be at least every 8 hours.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.
3701-31-04 (E)(3)(c) / Location of emergency telephone posted when the phone is not visible.
Repeat Sign indicating the location of the emergency device (telephone) was not provided and emergency device was not visible in the pool area.
Ensure that a sign informing patrons of the location of the emergency device is always visible in the pool area.
