Corrected During Inspection Secondary means of entrapment/evisceration prevention was not provided. Observed no VGB covers on equalizer lines and no plugs on port underneath skimmer bucket. PIC plugged all ports.
Submit plans to the Ohio Department of Health for review and approval for the secondary means that will be employed on the pool.
