Secondary means of entrapment/evisceration prevention was not provided. Observed no plugs at equalizer lines or VGB covers at pool side. PIC plugged equalizer line under skimmer weir. Ensure that equalizer lines are plugged or compliant covers are put in place.
Submit plans to the Ohio Department of Health for review and approval for the secondary means that will be employed on the pool.

Per conversation with pool operator, VGB covers are to be installed at equalizer lines.
Pool perimeter fence observed to have gap greater than four inches at corner near woods. Ensure that barrier is put in place to prevent a gap greater than four inches.
