[?] A summary of the violations found during the inspection are listed below.

3701-31-04 ( D )(6)(a) / ORP reading shall be 650 millivolts or greater.
Repeat ORP reading was below 650 mv
Adjust ORP calibration. If necessary, have the ACC unit inspected to determined why the ORP is low.
3701-31-04 (B)(1)(e) / Automatic Chemical Controller (ACC) shall function properly.
Critical Repeat Automatic Chemical Controller (ACC) was not functioning properly at the time of the inspection. Pressure switch / pump interlock was unplugged at the time of the inspection.
Activate the pressure switch / pump interlock for the ACC unit as required. Do not operate the pool when the ACC unit is not functioning properly. Ensure that the ACC unit is functioning properly prior to placing the pool back into operation.
3701-31-04 (B)(4) / The licensee /authorized representative is keeping records
Repeat The licensee /authorized representative was not keeping or maintaining operational records. At least once each week since the pool season began, one or more days have gone by without any test results being entered in the daily pool operation reports.
All facility records must be maintained.
3701-31-04 (B)(4)(a)(i) / Disinfection residuals are tested daily at pool side at required intervals for pools/automatic chemical controllers/special features
Repeat Disinfection residuals were not tested daily at pool side and/or at the required intervals for pools/automatic chemical controllers/special features. Test results missing at least once each week.
Test the disinfection residual every day before bathers enter the water. Disinfection residual must then be tested 12 hours while the pool is in use.
3701-31-04 (B)(4)(a)(ii) / Combined chlorine is tested daily at pool side prior to use
Repeat Combined chlorine was not tested daily at pool side prior to use. Readings are recorded in the daily pool operation reports, yet little if any Reagent #3 (required to determine total chlorine concentration and calculate combined chlorine concentration) has been used so far this season. At least once each week results are missing in the daily pool operation reports.
Test for combined chlorine by adding 5 drops of Reagent #3 to the free chlorine test sample and record actual test results prior to bathers entering the water and every 12 hours while the pool is in use.
3701-31-04 (B)(4)(a)(iii) / pH is tested daily at pool side at required intervals for pools/automatic chemical controllers/special features
Repeat pH was not tested daily at pool side and/or at required intervals for pools/automatic chemical controllers/special features. No test results in the folder for this week.
pH must be tested prior to bathers entering the water and every 4 hours the pool is open for use. If ACC is in use pH must be tested every 12 hours while the pool is in use. For spraygrounds and spray features, the pH must be tested at 1 special feature every 6 hours the sprayground or spray feature is in use.
3701-31-04 (B)(4)(a)(vi) / Total alkalinity is tested weekly
Repeat Total alkalinity was not tested weekly.
Test total alkalinity at least once/week.
3701-31-04 (B)(4)(a)(vii) / Cyanuric acid is tested weekly
Repeat Cyanuric acid was not tested weekly.
Test for cyanuric acid level at least once/week.
3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Repeat Required turnover rates were not met. Observed flow rateof 55 gpm has dropped significantly (~25 gpm) since last inspection.
Throttle flow so that it is maintained betwee 90-141 gpm for proper turnover. If necessary, have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.
3701-31-04 (D)(7)(d) / Flow or pressure switch with a shut off valve shall be provided and tested monthly.
Critical Repeat Flow or pressure switch for a shut off valve was not provided or the unit was not tested monthly. Flow / pressure switch is currently bypassed (unplugged).
Activate the pressure switch as required. Have the ACC unit serviced or replaced with a unit that has a flow or pressure switch with an automatic shut off valve. Provide records indicating that the unit is being tested monthly.
3701-31-04 (E)(2)(d) / First aid kit provided
Repeat Disposable gloves were not available in the first aid kit (located in the men's restroom at the time of the inspection) or anywhere on the premises.
Provide at least 1 pair of unused dispsoable gloves in the first aid kit at the pool at all times. Correct By: 22-Jul-2015
3701-31-04 (E)(2)(e) / Working emergency telephone/call box provided.
Repeat Emergency telephone was not working. Phone has no dial tone.
Repair or replace phone so as to ensure that the emergency device is functioning properly and can be used to call 911. Correct By: 22-Jul-2015

Comments
Noted: Pool water clarity and chemistry meet code requirements at this time.
Reinspection was rescheduled due to weather and scheduling conflicts. Despite receiving 11 extra days to correct violations noted on the previous report, none of the listed violations has been corrected. The emergency phone must be repaired or replaced and gloves must be made available in the first aid kit within 48 hours to avoid administrative action by Clermont County Public Health
