[?] A summary of the violations found during the inspection are listed below.

3701-31-04 (B)(1)(c) / Minimum disinfection residual shall be maintained as required.
Critical Corrected During Inspection Chlorine disinfectant residual was less than 1 ppm for the pool
Do not operate the pool when the disinfectant residuals are less than what is required. Ensure the chlorine disinfectant residual is at 1 ppm or above prior to placing the pool back into operation. Chlorine residual was raised to 1 ppm free and total chlorine.
3701-31-04 (B)(4)(a)(i) / Disinfection residuals are tested daily at pool side at required intervals for pools/automatic chemical controllers/special features
Disinfection residuals were not tested daily at pool side and/or at the required intervals for pools/automatic chemical controllers/special features. ACC not in use.
Test the disinfection residual every day before bathers enter the water. Disinfection residual must then be tested every 4 hours when an ACC is not in use. If an ACC is in use the disinfectant residual must be tested every 12 hours while the pool is in use.
3701-31-04 (B)(4)(a)(ii) / Combined chlorine is tested daily at pool side prior to use
Repeat Combined chlorine was not tested daily at pool side prior to use and every 4 hours afterwards. ACC not in use.
Test for combined chlorine prior to bathers entering the water and every 4 hours the pool is open for use. If ACC is in use combined chlorine must be tested every 12 hours while the pool is in use.
3701-31-04 (B)(4)(a)(iii) / pH is tested daily at pool side at required intervals for pools/automatic chemical controllers/special features
pH was not tested daily at pool side and/or at required intervals for pools/automatic chemical controllers/special features. ACC not in use.
pH must be tested prior to bathers entering the water and every 4 hours the pool is open for use. If ACC is in use pH must be tested every 12 hours while the pool is in use.
3701-31-04 (B)(6)(c) / Cross connection hazards shall be prevented.
Gutter was tied to backwash line from filter.
Remove all cross connection hazards from the pool area.
3701-31-04 (B)(6)(c)(i) / Backflow prevention devices shall be maintained.
Repeat No backflow prevention device was provided on fill hose.
Provide appropriate backflow prevention device on fill hose.
3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Repeat Required turnover rates were not met.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.
3701-31-04 (B)(6)(j) / All SVRS devices are maintained/ functional and tested
Critical SVRS device was not tested in accordance with the manufacturer's specifications.
Have the SVRS device tested immediately and maintain the test results on site.
3701-31-04 (B)(6)(o) / Ladders/stairs/steps are in good repair/slip resistent
Repeat Railing into pool was loose.
Repair such that railing is secure.
3701-31-04 (B)(6)(s) / Perimeter barrier is in good repair to prevent unauthorized entry
Corrected During Inspection Perimeter barrier had 7" gap.
Repair the perimeter barrier so that there are not any gaps greater than 4 inches anywhere along the barrier. Board was placed over gap such that there were not gaps greater than 4".
3701-31-04 (D)(7)(d) / Flow or pressure switch with a shut off valve shall be provided and tested monthly.
Critical Flow or pressure switch for a shut off valve was not tested monthly
Provide records indicating that the unit is being tested monthly.
