3701-31-04 (A) / Approval shall be obtained for any construction or alterations of a public swimming pool.
Repeat Observed the chemical disinfection feeder pump to have been replaced with a Mec-O-Matic, Dolphin Series Model No. US50-XA-SAUXXX from a FlexFlow Model No. A1N30A7T.
Complete and submit an Equipment Replacement Notification form for the disinfection feeder to ODH for review and approval.

Combined chlorine was not tested daily at pool side prior to use.
Test for combined chlorine prior to bathers entering the water and every 4 hours the pool is open for use. If ACC is in use combined chlorine must be tested every 12 hours while the pool is in use. For spraygrounds and spray features, the combined chlorine must be tested at 1 spray feature every 6 hours the sprayground or spray feature is in use.

Repeat Observed the pool deck to be in disrepair and crumbling by the pool main entry hand rail at the shallow end.
Repair areas of the deck where standing water is occurring. If more than 50% of the deck must be replaced, plans must be submitted to the Ohio Department of Health for review and approval.

Critical Repeat Corrected During Inspection Observed the door to the equipment and chemical storage area to be open at time of inspection. Room was accessible to individuals from the parking lot.
Ensure the equipment and chemical storage areas are accessible to authorized personnel only. Discussed with operator, door was shut and locked at time of inspection.
