Submit an equipment replacement notification form to the Ohio Dept. of Health for approval for the equipment change made.

Required turnover rates were not met. Flow rate measured at 60 GPM; 74 GPM required for 8 hour turnover; must not exceed filter max flow capacity of 141.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.
3701-31-04 (B)(6)(s) / Perimeter barrier is in good repair to prevent unauthorized entry
Perimeter fence had gaps greater than 4 inches along the base of the green chain link fence.
Repair the base / bottom of the green chain link perimeter barrier so that there are not any gaps greater than 4 inches anywhere along the barrier.
3701-31-04 (D)(7)(d) / Flow or pressure switch for a shut off valve provided and unit is tested monthly
Flow or pressure switch for a shut off valve was not provided or the unit was not tested monthly. ACC failed to register an alarm or interrupt chlorine feed during simulated flow interruption.
Have the ACC unit serviced or replaced with a unit that has a flow or pressure switch with an automatic shut off valve. Provide records indicating that the unit is being tested monthly.
3701-31-04 (E)(3)(b) / "Children must be supervised" or "Swimming alone is not recommended" signs posted
The unguarded pool was in use without the "Swimming alone is not recommended" sign(s) posted.
Post the "Swimming Alone is Not Recommended" sign(s) at all times the pool is available for use.
