3701-31-04 (B)(1)(c) / Minimum disinfection residual shall be maintained as required.
Critical Repeat Chlorine disinfectant residual was less than 1 ppm for the pool
Do not operate the pool when the disinfectant residuals are less than what is required. Ensure the chlorine disinfectant residual is at 1 ppm or above prior to placing the pool back into operation.

Water temperatures were not being recorded daily.
Take and record the water temperature at least once/day.
3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Repeat Required turnover rates were not met. Observed pool to be at a flow rate of 50gpm, which is below required flow rate to meet 480 minute turnover rate.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.

pH was outside the range of 7.2 to 7.8. Observed pH at 8.
Treat the pool water so that the pH is within the acceptable range.
