3701-31-04 ( D )(6)(a) / ORP reading shall be 650 millivolts or greater.

Adjust ORP calibration. If necessary, have the ACC unit inspected to determined why the ORP is low.
3701-31-04 (B)(1)(e) / Automatic Chemical Controller (ACC) shall function properly.
Critical Automatic Chemical Controller (ACC) was not functioning properly at the time of the inspection. Pressure switch / pump interlock was unplugged at the time of the inspection.
Activate the pressure switch / pump interlock for the ACC unit as required. Do not operate the pool when the ACC unit is not functioning properly. Ensure that the ACC unit is functioning properly prior to placing the pool back into operation.

The licensee /authorized representative was not keeping or maintaining operational records. There are no test results for this week beginning July 5 in the folder.
All facility records must be maintained and available for review upon request.
3701-31-04 (B)(4)(a)(i) / Disinfection residuals are tested daily at pool side at required intervals for pools/automatic chemical controllers/special features
Disinfection residuals were not tested daily at pool side and/or at the required intervals for pools/automatic chemical controllers/special features. No test results recorded so far this week.
Test the disinfection residual every day before bathers enter the water. Disinfection residual must then be tested every 4 hours when an ACC is not in use. If an ACC is in use the disinfectant residual must be tested every 12 hours while the pool is in use. For spraygrounds and spray features, the disinfectant residual must be tested at 1 spray feature every 6 hours the sprayground or spray feature is in use.
3701-31-04 (B)(4)(a)(ii) / Combined chlorine is tested daily at pool side prior to use
Combined chlorine was not tested daily at pool side prior to use. Readings are reecorded in the daily pool operation reports, yet little if any Reagent #3 (required to determine total chlorine concentration and calculate combined chlorine concentration) has been used so far this season. There are no test results in the folder for this week beginning July 5.
Test for combined chlorine by adding 5 drops of Reagent #3 to the free chlorine test sample and record actual test results prior to bathers entering the water and every 12 hours while the pool is in use.

pH must be tested prior to bathers entering the water and every 4 hours the pool is open for use. If ACC is in use pH must be tested every 12 hours while the pool is in use. For spraygrounds and spray features, the pH must be tested at 1 special feature every 6 hours the sprayground or spray feature is in use.

Repeat Required turnover rates were not met.Observed flow rate is approximately 10 gpm less than necessary to achieve 8 hour turnover rate.
Throttle flow so that it is maintained betwee 90-141 gpm for proper turnover. If necessary, have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.
3701-31-04 (D)(7)(d) / Flow or pressure switch with a shut off valve shall be provided and tested monthly.
Critical Flow or pressure switch for a shut off valve was not provided or the unit was not tested monthly. Flow / pressure switch is currently bypassed (unplugged).
Activate the pressure switch as required. Have the ACC unit serviced or replaced with a unit that has a flow or pressure switch with an automatic shut off valve. Provide records indicating that the unit is being tested monthly.

Repeat Disposable gloves were not available in the first aid kit (located in the women's restroom at the time of the inspection) as required.
Provide at least 1 pair of unused dispsoable gloves in the first aid kit at the pool at all times.

Emergency telephone was not working. Phone has no dial tone.
Repair or replace phone so as to ensure that the emergency device is functioning properly and can be used to call 911.

IMPORTANT NOTE: The location of the bicycle rack adjacent to the pool perimeter fence may be used by trespassers to gain access to the pool when it is closed. Strongly recommend relocating the bike rack to the other end of the parking lot so as to prevent unauthorized access when the pool is closed.

Other notes:
Water clarity, disinfectant residuals and water quality readings meet code requirements at this time.
Pool looks great! Record keeping needs to improve.
