A new disinfection system was installed.
Submit an Equipment Replacement Notification form for the disinfection system to the Ohio Department of Health for review and approval.

Required turnover rates were not met. Turn over rate with flow rate of 45 GPM is insufficient. Turn over rate is 23 hours and 6.6 minutes.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.

A USCG Type IV personal flotation device with 30 -60 foot throw line was not provided at the unguarded pool. Throw line was broken in places because it was brittle.
Provide a proper floatation device at all times. Replace throw line.
