Observed no automatic chemical controller (ACC) test record for last 30 days.
The test results/service records for these devices must be kept on site for review. If the device has not been recently serviced, have the device tested immediately and provide documentation of the test results. Ensure the ACC is tested once monthly and recorded on site. ACC sounded no flow alarm when tested.

Repeat Required turnover rates were not met. Flow rate of 120 gpm is below minimum required flow rate of 130 gpm that is required to produce an 8 hour turnover rate.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.
