Repeat Required turnover rates were not met.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.

Repeat Flow meter is not sized appropriately for pool.
Properly install a flow meter or have a pump curve available for every water pump used for the pool.

First aid equipment was not provided.
Provide first aid equipment, including a first aid kit, at the pool at all times.
3701-31-04 (E)(2)(e)/Working emergency telephone/call box provided.
Emergency device (telephone) was not working.
Ensure that the emergency device is functioning properly and rings to the emergency response authorities.
3701-31-04 ( D )(6)(a)/ORP reading shall be 650 millivolts or greater.
ORP reading was below 650 mv
Have the ACC unit inspected to determined why the ORP is low.
3701-31-04 (B)(4)(a)(vi)/Total alkalinity is tested weekly
Total alkalinity was not tested weekly.
Test total alkalinity at least once/week.
3701-31-04 (B)(4)(c)/SVRS/APSOS/ACC test results/ service records are maintained
ACC test results were not recorded weekly.
The test results/service records for these devices must be kept on site for review. If the device has not been recently serviced, have the device tested immediately and provide documentation of the test results. ACC was tested and worked (no flow).

Turnover rate cannot be determined due to flow meter not being sized correctly. However, flow rate was higher.
