3701-31-04 (A) / Approval shall be obtained for any construction or alterations of a public swimming pool.
Observed the chemical disinfection feeder pump to have been replaced with a Mec-O-Matic, Dolphin Series Model No. US50-XA-SAUXXX from a FlexFlow Model No. A1N30A7T.
Ensure an Equipment Replacement Notification form is completed and submitted to the Ohio Department of Health for review and approval.
3701-31-04 (B)(1)(a) / Outlet covers shall be installed/secured/in compliance
Critical Observed the equalizer lines to be operational and not properly covered with compliant covers. Outlet covers shall be secured at all times so they cannot be removed without the use of tools. Outlet covers shall meet the requirements of ANSI/ASME A112.19.8-2007. Equalizer lines shall have compliant covers or be removed or plugged.
Ensure the equalizer lines are properly covered, removed, disabled or plugged.

Outlet covers shall meet the requirements of ANSI/ASME A112.8-2007. Equalizer lines shall have compliant covers or be removed or plugged. Was informed the covers are in the equipment storage room.
Replace the equalizer line covers to prevent possible injury to bathers and to prevent possible entrapment hazards.

Observed the pool deck to be in disrepair and crumbling by the pool main entry hand rail at the shallow end.
Repair areas of the deck where standing water is occurring. If more than 50% of the deck must be replaced, plans must be submitted to the Ohio Department of Health for review and approval.

Repeat Observed both entry gates of the perimeter barrier not self-closing or latching. All perimeter barriers shall be with gates or doors that are self-closing and lockable.
Repair the gates so they are self-closing and latching.
3701-31-04 (B)(6)(t) / Equipment/chemical/storage areas are free of hazards/accessible only to authorized personnel.
Critical Observed the pool pump room and chemical storage area doors to be propped open and accessible to the public. All equipment, chemical, and other storage areas shall be maintained in a clean condition, free from any hazards, and accessible to authorized personnel only.
Ensure the doors are closed and areas maintained inaccessible to the public.

Repeat Sign indicating the location of the emergency device (telephone) was not provided and emergency device was not visible in the pool area.

INSPECTION NOTES:
--No current information on file regarding pool main drain covers. Submit certificates of compliance with make/model information with expiration dates for existing drain covers or replace drain covers and submit documentation to Clermont County Public Health.
--Equalizer lines observed to be operational and not properly covered. Equalizer lines are required to have compliant covers or be plugged requiring the use of tools to remove. Was informed by the pool operator that the equalizer line covers are in the equipment room. Install covers, plug or disable the equalizer lines to prevent possible injury to bathers and to prevent possible entrapment hazard. Recommend pool remain closed until equalizer lines are covered with compliant covers, plugged or disabled. Will follow-up.
--Ensure an Equipment Replacement Notification form is completed and submitted to the Ohio Department of Health for the change in chemical disinfection feeder.
