3701-31-04 (B)(1)(c) / Minimum disinfection residual shall be maintained as required.
Critical Chlorine disinfectant residual was less than 1 ppm for the pool. Pool operator was called and came to increase chlorine levels.
Do not operate the pool when the disinfectant residuals are less than what is required. Ensure the chlorine disinfectant residual is at 1 ppm or above prior to placing the pool back into operation. 3701-31-04 (B)(4)(b) / Life guard/ CPR/First Aid training certificates are current and available
Life guard/ CPR/First Aid training certificates were not available on site.
Provide documentation of all training certificates for all lifeguards. 3701-31-04 (B)(4)(c) / SVRS/APSOS/ACC test results/ service records are maintained
Records did not show if Vac Alert was tested monthly.
Test Vac Alert monthly to ensure it is properly functioning. 3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Required turnover rates were not met.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve. 3701-31-04 (D)(09) / Chemicals shall not be manually added to pool when bathers present.
Observed disinfectant pucks in skimmer basket.
Stop adding chemicals to the pool when bathers are present. 3701-31-04 (E)(2)(a)(i) / 12 ft. non-telescopic reach pole with shepherd's crook is provided
12 ft. non-telescopic reach pole with shepherd's crook was not provided at the unguarded pool. Observed pole but no hook.
Provide a 12 ft. non-telescopic reach pole in the pool area at all times. 3701-31-04 (E)(2)(a)(ii) / A USCG Type IV personal flotation device with 30 -60 foot throw line provided
A USCG Type IV personal flotation device with 30 -60 foot throw line was not provided.
Provide a proper floatation device at all times. 3701-31-04(D)(6)(a)(2) / Pool free chlorine disinfectant residual shall be above 1 ppm.
Critical Repeat Pool free chlorine disinfectant residual was below 1 ppm.
Do not operate the pool until the chlorine disinfectant residual is at 1 ppm or above.

Pool Notes: -Ensure that chlorine levels are at least 1ppm. Called Shane and he sent someone out to increase chlorine level. -Ensure that turnover rate is at or under 480 minutes, minimum flow should be 99 gpm. -Ensure pool records include that weeks date. Re-inspection to ensure pool disinfectant levels meet code requirement, all safety equipment is available, and pool is not hand dosing disinfectant in skimmer basket. |
