Repeat Equipment was replaced since last inspection. New ACC, chlorine feeder and pH feeder have been installed since last inspection.
Submit an equipment replacement notification form to the Ohio Dept. of Health for approval for the equipment change made.
3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Repeat Required turnover rates were not met. Flow rate measured at 60 GPM; 74 GPM required for 8 hour turnover; must not exceed filter max flow capacity of 141.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.
3701-31-04 (B)(6)(s) / Perimeter barrier is in good repair to prevent unauthorized entry
Repeat Perimeter fence can still be easily manipulated to allow gaps greater than 4 inches along the base of the green chain link fence.
Repair the base / bottom of the green chain link perimeter barrier so that gaps greater than 4 inches cannot be created anywhere along the barrier. This will minimally require securing the fence to the poles along the western perimeter, and securing the base of the fence along the south and west perimeter with either a pole (as is the case for the chain link fence along the walkway across from the building) or a high tension wire along the base so that it cannot be easily manipulated to allow unauthorized entry.

Critical Repeat Flow or pressure switch for a shut off valve was not provided or the unit was not tested monthly. ACC failed to interrupt chlorine feed during simulated flow interruption.
Have the ACC unit serviced or replaced with a unit that has a flow or pressure switch with an automatic shut off valve. Provide records indicating that the unit is being tested monthly.
