3701-31-04 (A) / All construction or alterations of a public swimming pool are approved
Repeat Pool Chlorinator was changed. Decking, pool backwash/waste line, and perimeter barrier (including door) were changed.
Submit Equipment change notification form to ODH. Submit plans to Ohio Department of Health regarding changes made to decking, perimeter barrier, and backwash/waste disposal.
3701-31-04 (B)(4)(c) / SVRS/APSOS/ACC test results/ service records are maintained
Repeat Corrected During Inspection ACC test results records were not maintained. According to operator, ACC was tested Monday of last week and not recorded.
The test results/service records for these devices must be kept on site for review. If the device has not been recently serviced, have the device tested immediately and provide documentation of the test results. ACC test was recorded.

Repeat Required turnover rates were not met.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.

Combined chlorine was not tested daily at pool side prior to use.
If ACC is in use combined chlorine must be tested every 12 hours while the pool is in use.

Total alkalinity was not tested weekly.

Door to pool does not latch and has no latch.
Operator has agreed to close pool until it can be ensured that door latches in addition to self closing.
Comments
- Sign with emergency phone number was provided in pool enclosure.
- Operator emailed on 7/20/17 and said that door latch had been corrected. Pool re-inspection was rescheduled to this date.
- Ensure plans as specified in violation 3701-31-04 (A) are provided to Ohio Department of Health such that changes made to facility can be approved.
- Door to enclosure self closed and self latched.
