3717-1-05.1(O)(2)
Critical Handwashing sink is being used for purposes other than handwashing. Observed handsink holdinf pitcher of water. Hand Sink is to only be used for handwashing. Sink can not be used to fill containers of liquids or store containers.

3717-1-03.2(K)
In-use utensils improperly stored. Observed scoop being stored in container containing pecans with handle in contact with product. Utensils are not permitted to be stored in this manner.

3717-1-04.8(E)(1)
Improper storage of single-service and single-use articles. Observed box of cups stored on floor of facility. Ensure that all supplies and articles are stored at least six inchs off the floor.

3717-1-04.5(A)(3)
Non-food contact surfaces of equipment are unclean. Observed residue build-up on bottom of reach in freezer located in rear of facility. Ice build-up in counter top freezer unit and debris on inside lid and side of ice machine. Clean these areas to sight and touch of visible debris.

Critical Insufficient air gap between the flood rim and the water supply inlet. Observed blender washer in three bowel sink basin. No back flow preventer observed on water line to unit. Backflow prevention must be provided to this location to prevent possible contamination of water system.

Critical Direct connection between the sewage system and a drain originating from equipment in which food is placed. Observed that drain line from side storage ice machine lacks required backflow prevention. Provide an air gap on drain line from unit to be compliant with plumbing and food code requirements.

Observed foul odor in preparation area of facility. Spoke with PIC regarding concern. Discussed drain/hole in floor that may be plumbing line that lacks proper water trap and allowing gasses to escape into prep area. Determine cause of foul odor in facility.

Premises not organized and cluttered. Rear storage areas, side storage area need cleaned and organized to allow for effective cleaning and pest control. Have all items properly stored and organized.
