3701-31-04 (B)(4)(a)(ii) Combined chlorine was not tested daily at pool side prior to use. Test for combined chlorine prior to bathers entering the water and every 4 hours the pool is open for use. If ACC is in use combined chlorine must be tested every 12 hours while the pool is in use. For spraygrounds and spray features, the combined chlorine must be tested at 1 spray feature every 6 hours the sprayground or spray feature is in use.

3701-31-04 (B)(4)(a)(vi) Total alkalinity was not tested weekly. Test total alkalinity at least once/week.

3701-31-04 (B)(4)(a)(vii) Cyanuric acid was not tested weekly. Test for cyanuric acid level at least once/week.

3701-31-04 (B)(6)(h) Required meters and gauges were not maintained or were not functional while the pool is in operation. Maintain all meters and gauges in a properly funtioning manner at all times while the pool is in operation. (FLOW METER WAS NOT FUNCTIONING. ACCORDING TO POOL MANAGER ONE HAS BEEN ORDERED.)

3701-31-04 (E)(2)(a)(ii) A USCG Type IV personal flotation device with 30 -60 foot throw line was not provided at the unguarded pool. Provide a proper floatation device at all times. (The life buoy must have a one-quarter inch diameter line not less than 30 feet or more than 60 feet in length.)

Pool chemistry was performed along with pool manager at her request because she wanted to have a better understanding of process. New pool record forms were provided and reviewed at inspection - this form needs to be used to document the required pool chemistry. New incident reports were provided and were reviewed. All safety equipment must be out and available when pool is open to bathers - spine board, life buoy, shephard's crook. A sign needs to be posted stating where the emergency phone is located. For example "EMERGENCY PHONE IS LOCATED AT THE CONCESSION STAND WINDOW"
