3701-31-04 (A)
Repeat A new circulation pump, filters, and chlorine feeder were added without submitting an Equipment Replacement Notification form to Ohio Department of Health. Submit a form for each piece of equipment.
Contact ODH to obtain approval for any construction/alterations before operating pool.
3701-31-04 (E)(4)(e)(iv)
Lifeguards were not easily identified by bathers or other lifeguards.
Provide swimwear or require that lifeguards were similar attire so they are easily identified.

Lifeguards did not have a CPR pocket mask on their person. LIfeguards were not aware of the location of the CPR pocket mask.
Provide lifeguards with CPR pocket masks and rescue tubes and require that these items be with the lifeguard at all times while on duty.
Comments
Provide the pool testing records by email. Reminder that Free Chlorine, Combined Chlorine, pH need to be tested and recorded prior to bather entry and at least every 12 hours while in use. Water clarity and temperature need to be recorded daily and total alkalinity recorded at least weekly. These need to be recorded and kept at minimum for 2 years and reviewable during inspections.

Make sure lifeguards are similarly attired and prepared to immediately enter the water.
