3701-31-04 (A)
Equipment, at a public swimming pool, was changed/altered without obtaining approval from the Ohio Department of Health (ODH) by submitting an Equipment Replacement Notification (ERN). (THE HAYWARD PUMP THAT IS CURRENTLY INSTALLED DOES NOT HAVE ODH APPROVAL AND AN EQUIPMENT REPLACEMENT NOTIFICATION FORM MUST BE SUBMITTED. AND ERN WAS PROVIDED)
Obtain approval from the Ohio Department of Health (ODH) by submitting an Equipment Replacement Notification (ERN) before operating pool.
3701-31-04 (B)(1)(c)
Critical Chlorine disinfectant residual was less than 1 ppm for the pool (AT TIME OF INSPECTION FREE CHLORINE WAS AT .5 ppm)
Do not operate the pool when the disinfectant residuals are less than what is required. Ensure the chlorine disinfectant residual is at 1 ppm or above prior to placing the pool back into operation.
3701-31-04 (B)(4)(a)(i)
Disinfection residuals were not tested daily at pool side and/or at the required intervals for pools/automatic chemical controllers/special features. (AT TIME OF INSPECTION IT WAS OBSERVED THAT NO POOL CHEMISTRY IS NOT BEING PERFORMED AND DOCUMENTED. POOL CHEMISTRY MUST BE CONDUCTED EVERY DAY, PRIOR TO BATHERS ENTERING THE POOL AND EVERY 12 HOURS THAT THE POOL IS OPEN TO BATHERS.)
Test the disinfection residual every day before bathers enter the water. Disinfection residual must then be tested every 4 hours when an ACC is not in use. If an ACC is in use the disinfectant residual must be tested every 12 hours while the pool is in use. For spraygrounds and spray features, the disinfectant residual must be tested at 1 spray feature every 6 hours the sprayground or spray feature is in use.

SVRS/APSOS/ACC test results/ service records were not maintained.
The test results/service records for these devices must be kept on site for review. If the device has not been recently serviced, have the device tested immediately and provide documentation of the test results.

Backwash lines and lines from drainage sumps did not have an air gap prior to discharge (AT TIME OF INSPECTION THERE WAS NO AIR GAP ON THE DRAIN LINE)
Open the lines that discharge into the backwash or drainage sumps so that there is an air gap and the lines can drain freely.

Test kit reagents were not fresh or stored properly.

3701-31-04 (E)(2)(a)(ii)
A USCG Type IV personal flotation device with 30 -60 foot throw line was not provided at the unguarded pool. (AT TIME OF INSPECTION THE EAST LIFE BOUY DID NOT HAVE THE ROPE ATTACHED)
Provide a proper floatation device at all times.

Emergency device (telephone) was not working.
Ensure that the emergency device is functioning properly and rings to the emergency response authorities.
Comments
Pool closed due to insufficient free chlorine (disinfectant). Hampton cannot reopen pool until Hancock Public Health has conducted a passing inspection. Call when pool has sufficient disinfectant for a reinspection. Please correct all violations prior to reinspection.

~Pool records must be better maintained. Month and date is required on the pool records. If the pool is closed, the pool records must be marked as such. If the pool is open, then pool chemistry MUST BE CONDUCTED. IF THERE IS NO ONE TO PERFORM POOL CHEMISTRY, THE POOL MUST BE CLOSED.
