ORP reading was below 650 mv (AT TIME OF INSPECTION THE ACC SHOWED NO READING FOR ORP)
Have the ACC unit inspected to determined why the ORP is low.
3701-31-04 (B)(4)
The licensee /authorized representative was not keeping or maintaining operational records. (AT TIME OF INSPECTION RECORDS COULD NOT BE LOCATED THAT SHOWED WATER CHEMISTRY HAD BEEN PERFORMED TODAY. ALSO, RECORDS MUST BE ACCURATE TO INDICATE DAY, MONTH AND YEAR. IF THE POOL IS CLOSED, RECORDS MUST BE MARKED AS CLOSED)
All facility records must be maintained and available for review upon request.
3701-31-04 (B)(4)(a)(i)
Disinfection residuals were not tested daily at pool side and/or at the required intervals for pools/automatic chemical controllers/special features.
Test the disinfection residual every day before bathers enter the water. Disinfection residual must then be tested every 4 hours when an ACC is not in use. If an ACC is in use the disinfectant residual must be tested every 12 hours while the pool is in use. For spraygrounds and spray features, the disinfectant residual must be tested at 1 spray feature every 6 hours the sprayground or spray feature is in use.

Combined chlorine was not tested daily at pool side prior to use.
Test for combined chlorine prior to bathers entering the water and every 4 hours the pool is open for use. If ACC is in use combined chlorine must be tested every 12 hours while the pool is in use. For spraygrounds and spray features, the combined chlorine must be tested at 1 spray feature every 6 hours the sprayground or spray feature is in use.

Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.

pH was outside the range of 7.2 to 7.8.
Treat the pool water so that the pH is within the acceptable range.

Test kit reagents were not fresh or stored properly. (AT TIME OF INSPECTION THE MAJORITY OF THE TEST KIT REAGENTS WERE EXPIRED)
Obtain fresh reagents and store them properly.

Combined chlorine was greater than 1.0 ppm.
Treat the pool so that the combined chlorine is less than 1 ppm.
Comments
At the time of this inspection the pool operator was not present. I was informed by the front desk clerk that the pool operator was present this morning to perform pool water chemistry. Records were unable to be found in the pool equipment room that showed this to be true. Old pool records were found. Pool chemistry must be performed and documented prior to bathers entering pool (prior to opening hours) and every 12 hours the pool is open. Records must be available and able to obtained at all times. Records must be maintained for a period of 2 years to be in compliance, meaning that records older than 2 years can be disposed of.
