| 3701-31-04 (A) Equipment, at a public swimming pool, was changed/altered without obtaining approval from the Ohio Department of Health (ODH) by submitting an Equipment Replacement Notification (ERN). Last documented chlorine feeder when the spa was operational was a Stenner 45M5. Current chlorine feeder is Stenner 85M5. Jet pump for the spa was replaced. Previous pump was a 3HP StaRite P6E6H-209L. New pump is a 3HP StaRite Intellipro3 VSF P/N 356230. Submit ERN for new equipment. Obtain approval from the Ohio Department of Health (ODH) by submitting an Equipment Replacement Notification (ERN) before operating pool.  |
| 3701-31-04 (B)(6) Equipment was not maintained per manufacturers specifications. The flow of the circulation pump is above the max flow rate of the sand filter. Flow rate was above 70 gpm and the max flow rate for the filter is 68.2 gpm. Reduce flow rate to below 68.2 gpm. |
